Employee alleges they were changing a drill bit and the drill fell from the hammer and pinned the index and ring fingers under the drill bit causing injuries to their right hand. Employee also received stitches.

Employee was doing maintenance at the plant, removing a section of catwalk. While doing so, the catwalk fell hitting EE's wrist pushing it into the hood of the scalping screen causing laceration on the outside of EE's rt wrist.
Employee was belt splicing when EE reached for the skiver tool by twisting backward to the right, reaching with EE's right hand/arm. Once EE had hold of tool EE realized it was hung on a piece of belting, EE started pulling and jerking to free up the tool when EE heard pop from shoulder and felt pain.

Employee was lifting a full argon gas cylinder for the welder. Felt pain in lower back. Returned 1/08/21 - didn't report until 1/11/2021 - at that time we sent EE to the Doctor.

Employee was working on a conveyor scraper. While tapping on scraper bracket with hammer, struck right forefinger knuckle against bracket causing abrasion. EE did not initially report injury, thought it to be minor, on Sunday 9/20 it was infected and sought treatment @ urgent care.
Employee was working on the shop floor underneath the Komatsu 900 pit loader. When coming up from under machine EE hit head on the edge of the hydraulic tank causing a laceration on the top of EE's head, requiring stitches.

Employee was using a utility knife to cut a piece of conveyor belting. In the process the blade pulled from the belting, the knife struck EE's right leg which caused a laceration that required sutures.
employee was walking on crusher catwalk after crusher went down, stepped on rock and rolled ankle causing ligament strain of right ankle.
Employee arrived for the shift, while walking across the wet limestone parking lot EE slipped and fell onto knees, causing contusion on L knee. At time of this injury on 9/3/19, EE thought was okay. On 10/24/19 EE reported knee still bothering EE. Was sent to Dr for 1st visit. On 11/4/19 EE had follow-up & Dr put EE on restrictions.

Employee was at the crusher spraying inside of chute with 115 psi. The hose tip was just inside the chute door. Something broke loose above and it fell down hitting tip of the hose which caused employee's arm to go downward causing bone fractures in left arm.
Employee was going up steps of skid steer to pull the keys from the ingnition for the day. After retreiving the key, EE pivoted on EE's foot causing the boot tread to get caught in the step of the machine. Employee was thrown off balance and fell approx 2' to ground on right shoulder.
Employee was using pick axe to clear chute build up. The pick axe hit hard build up and slid off going deeper into chute, causing employee's finger to strike the chute hinge causing laceration.

EMPLOYEE WAS HOLDING IN POSITION A PIECE OF WATER PIPE ON ONE END AND THE CRANE WAS HOLDING THE OTHER SO THEY COULD BE WELDED TOGETHER. THE END THE EMPLOYEE WAS HOLDING DROPPED DOWN 6"" TO THE GROUND CAUSING THE EMPLOYEE'S PINKY FINGER TO GET BETWEEN THE PIPE AND THE GROUND, CAUSING LACERATION.
Employee was using wheel grinder on a piece of plate steel. Foreign object in right eye.
Employee was using a 6' pry bar on the hood at the shaker. The pry bar slipped and caused him to lunge forward on the catwalk approx 10' falling against the lower hand rail.
Employee was removing bent conveyor for repairs. While using a 12' x 12' timber to block the conveyor, the timber was pushed to far on one end causing his end to drop down onto angle iron which caught his pinky on right hand, which partially tore the fingernail from the nailbed.
Employee was getting off his excavator when he noticed there was mud on the step. To avoid slipping from the step he jumped from the track approx 3' landing on his left heel, which in turn caused pain in his back and leg.
Employee was working on snow removal with his highlift when he struck a piece of frozen ice and it jarred him in his machine causing pain in his lower back. EMPLOYEE RETURNED TO WORK ON 1/28 & 1/29/09....FIRST DAY MISSED 1/30/09

EMPLOYEE WAS PUTTING A CONVEYOR BELT ON WITH A ROPE ATTACHED TO A PULLEY THAT WAS THEN ATTACHED TO A TRUCK TO PULL. THE PULLEY BROKE AND THE ROPE HIT THE EMPLOYEE IN THE FOOT/ANKLE (R).
EMPLOYEE WAS HAMMERING ON WOOD BLOCK TO REMOVE SHAFT FROM THE SHAKER. MEANWHILE, THE SHAFT BREAKS LOOSE FALLING ONTO EMPLOYEE'S (L) LEG PINNING IT BETWEEN THE SHAFT AND THE DIVERTER PLATE.
Employee was using his left hand to hold the ""raise bed"" switch on his loaded haul truck. His hand slipped from the switch causing his bed to free fall approx 1-2' causing the employee's body to jar.
EMPLOYEE WAS AT THE PLANT INSTALLING DUST SHIELD OVER THE CONVEYOR WHEN HE FELT A SHARP PAIN IN HIS LEFT CALF. POSSIBLE REPETITIVE MOTION FOR THE PAST 2 DAYS APPROX 5 HRS/DAY STANDING ON THE BALL OF HIS FOOT.
For the past 6 days for 2-3 hrs per day, this employee has been lying on his back at the crusher cleaning tar paper off the screens (from crushing asphalt) with his arms in the air. The repetive motion of holding arms up caused a golf ball size lump to pop up on his shoulder at the joint of the arm.
Employee was operating a skidsteer cleaning up at the crushing plant. When he got out of the machine he felt a pain in his lower left back. (Not reported until 1/07/08).

EMPLOYEE WAS IN A MAN LIFT TURNING ANGLE BRACKETS IN THE UPRIGHT POSITION TO SET THE CONVEYOR FRAME ONTO THE BRACKETS. THE OPERATOR HOLDING THE END OF FRAME WITH FORKLIFT MOVED AND IT CAUSED THE CONVEYOR FRAME TO MOVE AND LET CONVEYOR COME DOWN , RESTING UPON EMPLOYEE'S LEFT THUMB.
This over the road driver got out of his truck @ stockpile (restricted area) while waiting to be loaded to air up his low tire. On the ground he reaches into his truck to turn on a valve to put compressed air to his air hose. He then accidentally released his parking brake which allowed the truck to roll back and his leg ended up under the front wheel of his truck.
Employee was climbing off a D8R Cat Dozer and lost his footing. Employee's right ankle hit the dozers blade arm and a step welded to the blade arm , causing bruising and swelling.

Employee was making a J bolt out of a concrete anchor. Instead of using a hammer he used a 2"" steel pin to strike the bolt. While hammering he slipped causing him to slide his little finger into the bolt causing laceration.
EE WAS CHANGING SCREEN CLOTH AT THE CRUSHER, STEPPED ON THE DISCHARGE LIP CAUSING EE TO SLIP AND LOSE FOOTING. EE SLIPPED DOWN THE DISCHARGE SHOOT APPROX 6' LANDING ON THE CONVEYOR BELT.

EE WAS REPLACING THE BEARING ON THE BEND ROLLER. THE CABLE HOLDING THE ROLLER SLIPPED CAUSING IT TO HIT HIS FINGER CAUSING A LACERATION.
EE WAS CLIMBING THE LADDER TO GET IN HIS TRUCK WHEN HIS HANDS SLIPPED FROM THE HAND RAILS, CAUSING HIM TO FALL TO THE GROUND.
EMPLOYEE WAS USING CUTTING TORCH TO CUT THE LEGS OFF A CONVEYOR. THE LEG FELL DOWN HITTING EMPLOYEE ON THE RIGHT SIDE OF THE FACE AND THE EAR. THE RIGHT EAR RECEIVED A LACERATION AS WELL AS A BURN FROM THE HOT METAL.
EE WAS STANDING ON A LADDER, UP APPROX. 6', WHEN HE OVER EXTENDED, CAUSING THE LADDER TO FALL TO THE SIDE. EE HIT HIS SHIN.
