Employee was walking on a grade when they slipped and fell on compacted snow. The employee became sore at the time of the fall. Employee went to the ER later that day on January 10th and was checked out and released. Employee went to the ER on January 26th at 12:46 a.m. and was then placed off of work.

Showing all 17Operator was climbing up dozer to enter the cab when EE lost EE's footing and slipped on the step causing EE to fall and hit EE's mouth on the handrail. Operator chipped EE's front tooth on the handrail. Steps were wet from the rain the previous night.
Miner missed a step climbing onto a locomotive.
Miner stepped off railcar injuring right knee on 7/31/19. EE reported the injury to operator on 8/3/19. EE has remained on full duty, per doctor orders, until they had surgery on 10/1/19 at which time EE was taken off duty until deemed fit to return.
The employee was climbing a step-ladder when employee fell causing injury.
Foreman inspecting plant when he fell landing on his right hand. He first sought diagnostic evaluation Friday, 03/25, and was referred to another clinic for additional evaluation, which was Thursday, 03/24. This revealed a broken bone in the heel of the hand. The Orthopaedic said it would heal with no treatment. There are no restrictions and no lost time.
Driver for White River Petroleum Co. was attempting to deliver diesel for the over-the-road tank. He was apparently using the access ladder and fell striking the top of the containment wall. This wall is (23") in height, and the top rung of the ladder is (54") from the containment floor. This person has (6) fractured ribs and will likely miss at least four weeks.
Employee walked up the hill to the office and then started walking down the hill back to the jobsite. He slipped and fell and as he fell his knee popped causing injury. EE is being diagnosed by a physician so that he can return to full duty.
picked up a piece of steel grating and turned as he stepped on a small amount of spilled and muddy 140 mesh limestone...he caught himself and prevented a fall but appearently wrenched his lower back resulting in a muscle strain...this has resulted in light duty and physical therapy...progressing very well

W/COMPANY DR. 2/2/05 RESULTED IN A MRI 2/3/05, THE RESULTS WERE READ ONLY TODAY. HE HAS A CRACKED PATELLA RT. KNEE & HAS AN APPT. W/AN ORTHOPEDIC DR. IN A FEW DAYS. WILL KEEP WORKING.
EMPLOYEE WAS ASSISTING WITH THE UNLOADING OF COAL/COKE FUEL CARS AT THE QUARRY LOCATION. HE ATTEMPTED TO CLOSE A RAILCAR DOOR USING HIS RIGHT FOOT TO PERFORM THIS TASK. HE PLANTED HIS LEFT FOOT & WHEN HE RAISED HIS RIGHT FOOT HIS KNEE BUCKLED RESULTING IN A STRAIN. EE HAS BEEN OFF WORK SINCE ELECTING TO TAKE A VACATION. Dr. SAID HE COULD RETURN 5/2/04 UNRESTRICTED.
EE WAS ON TOP OF QUICKLIME STORAGE/LOADOUT TO MEASURE TANKS, HE SLIPPED ON ICE BUT CAUGHT HIMSELF AVOIDING A FALL. EE APPARENTLY WRENCHED HIS BACK DURING THIS PROCESS & MISSED WORK ON 2/28/03. EE WAS ALIGNING LOADEING SHUTE OVER OPEN HATCH OF RAILCAR WHEN HE SLIPPED ON THE TOP OF THE CAR AND FELL WITH HAND EXTENDED TO BRAKE THE FALL. HE LANDED ON THE TOP OF THE CAR BUT BROKE HIS RIG HT HAND ONE BONE.
EE WAS LOADING BAG TRUCK...SNOW WAS FALLING VERY HEAVILY...WHILE EXITING TRUCK HE SLIPPED RESULTING IN A SPRAINED ANKLE/FOOT.
