EE slipped and fell to ground, at the end of the shift, the ee's ankle was swollen and painful. EE took the following day off of work.
WHILE WALKING FROM SCREEN TO PLATFORM WET BOOT SLIPPED , EE PIVOTED AND FELL TO A CONVEYOR BELT.PULLEYS ON CONVEYOR BRUISED RIGHT SHOULDER AND LOWER BACK. EE STATED THAT HE WAS OK AND DID NOT WANT TO SEE A DOCTOR ON 4-2-03 AND ALSO ON 4-7-03 ON 4-18-03 EE REQUESTED TO SEE A DOCTOR AND WAS SEEN THE SAME DAY.
