While attempting to climb into a front end loader(CAT 966H),an employee slipped, hurting both knees. The employee was climbing in to the front end loader to load the hoppers with barite ore. The employee stated that as he was e entering the cab, his left leg slipped causing his right knee to impact the top of the cab landing.

Maintenance man cut grating section from crushing area. Left for lunch after lunch went back to crushing area to finish job and fell through section that was cut out.

EE left his tools on barge he was loading earlier. Went down the gangway to pick up tools and a tug with 6 barges ran into dock and barge he was on shifted he fell to barge. Right knee started to swell that morning. 2 weeks later he was starting to load another barge & picked up on gangway to put on barge when his knee failed & he fell to the ground. Happened on 8/10/11.

BHDF employee was on top of loader #30 replacing a hydraulic line on the right tilt arm at the Argenta Mine. When the employee completed replacing the hydraulic line the employee stepped down onto the back of the bucket like he has done in the past for this particular job. Then the employee stepped off the bucket onto the ground and twisted his right knee.

Employee had set the brake on a railcar. When he climbed down from the railcar he stepped in a snow covered hole twisting his ankle. There was a snow storm at the time and visability was limited.

Employee was performing a walk a round inspection when he decided to cross a patch of ice. His left leg splipped and caused him to fall to the ground. He reported pain in his left knee.

Employee was walking to his vehicle at the end of his shift. He slipped on the snow and ice in the parking lot. The employee slipped backwards and fell on his back injuring the back of his head. His head was seeping blood from a cut about an inch long. He received medical treatment at the ER. He received six stitches. The employee was given full release to go back to work.

Employee was closing mine gate, when he slipped on the snow and ice, falling on his right side. He was admitted at the ER where he received a chest x-ray, muscle relaxer shot, and pain medications during the visit. He had no fractures, only a pulled muscle. He was given full release to return to work.

Employee injured his back when he stepped off the last rung of a 966G CAT front end loader ladder. When the employee reached the bottom rung of the ladder he released his hand hold and stepped off with his left leg. He reported feeling a sharp pain on the left side of his back and found it difficult to put pressure on his left leg.

Employee was moving a railcar from the loading area to couple to the next railcar in line. He climbed on the car to release the brake. When he dismounted the railcar, he stepped on the track and his foot slipped causing his right ankle to develop a hair line fracture.

PREPARING TO LOAD BARGE, HE STEPPED ON GRATING WALKWAY WITH LEFT FOOT. AS HE WAS PUTTING RIGHT FOOT ON GRATING, THE LEFT FOOT SLIPPED OFF CAUSING HIM TO FALL FORWARD. HE PUT OUT HIS HANDS SO HE WOULDN'T HIT HIS FACE ON THE WALKWAY, CUTTING HIS HANDS.

EE alleges that as he opened the door of the mill office with his left hand, he stepped in and planted his right foot first. As he did, his foot slipped and twisted and his whole body twisted left and he lost his balance and fell with his back against the door.
