EE WAS TIGHTENING A NUT ON TOP OF THE CORE OR THE CRUSHER. HE HAD ATTACHED A LARGE WRENCH TO THE NUT AND WAS STANDING ON TOP OF THE WRENCH WHILE SWINGING A SLEDGE HAMMER AT THE END OF THE WREN CH TO TIGHTEN THE NUT. HE SWUNG AND MISSED AND THE MEMENTUM OF HIS SWING CAUSED HIM TO LOSE HIS BALANCE AND FALL HE FELL ON TOP OF THE BOLTS THAT HOLD THE CRUSHER DOWN.
