Employee was servicing a loader. While working on the work platform of the loader, Employee mis-stepped and employee's foot left the platform and stepped down onto the top rung of the ladder, causing employee to lose balance falling 70" down onto a concrete floor. The employee landed on employee's feet, however sustained injuries to left foot and hip.
Employee was checking oil in hauler and was standing on steps. They slipped and struck diesel fill nozzle with right wrist. EE suffered undiagnosed non-specific wrist injury. EE was placed on restricted duty.
Employee dismounted from cab of hauler and planted left foot at an angle resulting in foot strain.
Employee tripped over rug outside of Scalehouse and fell. Striking head. Employee suffered head laceration and required 4 staples

Employee was cutting hinge on engine excavator cover and fell off
DRIVER ARRIVED @3:45PM, DROPPED EMPTY CONTAINER, HOOKED UP FULL CONTAINER, @4PM FELL WHILE WALKING BACK TO THE TRUCK. @4:14PM DRIVER USED CELL PHONE TO CONTACT COMPANY OFFICE REGARDING ACCIDENT. INJURY TO HEAD, NECK, SHOULDER, ARMS, UPPER BODY. TAKEN TO HERSHEY MEDICAL BY AMBULANCE FOR TREATMENT. RELEASED FROM E/R AFTER CT SCAN AND STITCHES. FOLLOW UP WITH FAMILY DOCTOR.
Employee slipped on snow covered ice and sprained left knee.
Employee slipped while climbing down a ladder and fell onto shoulder.
Employee was installing a new hose under crusher CR-3. The hose became stuck and the employee pulled on the hose to free it. The hose broke loose and the employee lost his footing and fell back against the side of the crusher. Employee suffered back strain.
Employee did not follow procedure and got out of truck violating posted rule. He twisted left knee. Employer became aware of recordable status on 06/15/12.
While working at PI-G Pump, setting up a level detector the EE stepped down on a pipe then stepped on to a flat base and when stepping off the base onto the ground he lost balance and twisted his ankle and knee.

EE WAS WASHING THE WINDSHIELD OF A HAUL TRUCK. HE LOST HIS BALANCE AND FELL TO THE GROUND, SUFFERING A COMPOUND FRACTURE OF THE RIGHT ANKLE AND EVULSION TYPE INJURY TO HIS RIGHT INDEX FINGER AND LACERATIONS ON THE LEFT INDEX FINGER.
