EE was operating a CAT 775F Haul Truck to the loadout area in the quarry pit from the crusher. Once at the loading area EE made the turn to get turned around however at this time, EE lost consciousness and hit the berm which stopped the truck from traveling over the highwall. The Loader Operator got out of the loader and climbed into the haul truck to shut it off.

Employee's hard hat fell off while leaning over rail. As EE was walking down steps to meet Supervisor who brought EE a new hard had, rock flew from crusher and struck employee on back of head.

Working in Pumping Station. Started to install 18"" pipe with 90 on end, 7' long. Put pipe into position, started to leave it down, when hook on lug was noticed; the chain was under the hook. Chain came around hook, pipe jolted down, sprung back up and then down again, busting the rigging. Employee grabbed the pipe and smashed his thumb.

DRIVER ARRIVED @3:45PM, DROPPED EMPTY CONTAINER, HOOKED UP FULL CONTAINER, @4PM FELL WHILE WALKING BACK TO THE TRUCK. @4:14PM DRIVER USED CELL PHONE TO CONTACT COMPANY OFFICE REGARDING ACCIDENT. INJURY TO HEAD, NECK, SHOULDER, ARMS, UPPER BODY. TAKEN TO HERSHEY MEDICAL BY AMBULANCE FOR TREATMENT. RELEASED FROM E/R AFTER CT SCAN AND STITCHES. FOLLOW UP WITH FAMILY DOCTOR.

Employee worked 6/14 and 6/15 with no report of any injury. Was a no call/no show for work on Monday 6/17/13. Reported to work on 6/18/13 and said he might have hurt his back on Friday while lifting drill bits into pick up truck with another employee.

THE EE WAS MOVING A SMALL 44"" DIAMETER STEEL PLATE WEIGHING ABOUT 60 LBS. WHEN HE PICKED UP THE PLATE HE FELT A POPPING SENSATION IN HIS BACK, BUT CONTINUED TO CARRY THE PLATE APPROX 50 FT. TH E INJURY WAS A RESULT OF IMPROPER BODY MECHANICSWHILE LIFTING. THE PLATE SHOULD HAVE BEEN CARRIED IN FRONT OF THE BODY RATHER THAN TO THE BACKSIDE.

EE WAS WASHING THE WINDSHIELD OF A HAUL TRUCK. HE LOST HIS BALANCE AND FELL TO THE GROUND, SUFFERING A COMPOUND FRACTURE OF THE RIGHT ANKLE AND EVULSION TYPE INJURY TO HIS RIGHT INDEX FINGER AND LACERATIONS ON THE LEFT INDEX FINGER.
