EE was operating a CAT 775F Haul Truck to the loadout area in the quarry pit from the crusher. Once at the loading area EE made the turn to get turned around however at this time, EE lost consciousness and hit the berm which stopped the truck from traveling over the highwall. The Loader Operator got out of the loader and climbed into the haul truck to shut it off.

Employee pulled on plate and felt pain in forearm. Was not made aware it was reportable till 6/23/14.
Working in Pumping Station. Started to install 18"" pipe with 90 on end, 7' long. Put pipe into position, started to leave it down, when hook on lug was noticed; the chain was under the hook. Chain came around hook, pipe jolted down, sprung back up and then down again, busting the rigging. Employee grabbed the pipe and smashed his thumb.

Employee worked 6/14 and 6/15 with no report of any injury. Was a no call/no show for work on Monday 6/17/13. Reported to work on 6/18/13 and said he might have hurt his back on Friday while lifting drill bits into pick up truck with another employee.
Driller was hammering on drill bit and metal shard flew off and hit employees hand

Employee failed to release pressure in hydraulic hose prior to removing. Pressure caused hose to break loose and struck employee on the right thigh.
Employee pinched 2nd finger on left hand between roller foot and upper rail of man basket. Required 3 stitches.
While cutting tree down the chainsaw came through the tree and hit the left leg.
Employee was sitting in truck while being loaded and complained of chest pain. 911 was called and employee was taken from site.
Employee did not follow procedure and got out of truck violating posted rule. He twisted left knee. Employer became aware of recordable status on 06/15/12.
While working at PI-G Pump, setting up a level detector the EE stepped down on a pipe then stepped on to a flat base and when stepping off the base onto the ground he lost balance and twisted his ankle and knee.
Employee was splicing conveyor belt and blade on utility knife broke off and cut finger.

THE EE WAS MOVING A SMALL 44"" DIAMETER STEEL PLATE WEIGHING ABOUT 60 LBS. WHEN HE PICKED UP THE PLATE HE FELT A POPPING SENSATION IN HIS BACK, BUT CONTINUED TO CARRY THE PLATE APPROX 50 FT. TH E INJURY WAS A RESULT OF IMPROPER BODY MECHANICSWHILE LIFTING. THE PLATE SHOULD HAVE BEEN CARRIED IN FRONT OF THE BODY RATHER THAN TO THE BACKSIDE.

EE WAS WASHING THE WINDSHIELD OF A HAUL TRUCK. HE LOST HIS BALANCE AND FELL TO THE GROUND, SUFFERING A COMPOUND FRACTURE OF THE RIGHT ANKLE AND EVULSION TYPE INJURY TO HIS RIGHT INDEX FINGER AND LACERATIONS ON THE LEFT INDEX FINGER.
