Showing all 8Employee left EE's work station to shut off a piece of equipment. Slipped on a spot of oil on floor and fell.
Employee working on crusher repairs coming down ladder at end of shift slipped on second to last rung of ladder, stumbled and hit/fell into air compressor tongue resulting in bruised left flank.
Employee slipped on wet/muddy marble and caught himself before he landed on the ground, pulled himself up quickly. Complained of lower back pain whether moving or not.
Employee was attempting to move bar saw track with a pry bar. The pry bar slipped, causing employee to lose his balance and fall forward, hitting right knee on the track.
EE fell. EE was measuring a slab located on A-frame. Right foot got stuck on the A-frame and without EE realizing it EE fell when he went to move.
EE was operating an automated polishing line. The hose which supplies water to it came off. He cut off all electrical, then went to back of machine to cut off water source where he slipped and knee buckled causing fall.
No conditions contributing to accident. Tripped while stepping up to platform. Became unbalanced and fell on right leg, then both knees.
