Showing all 5Maintenance employee was working on an aggregate discharge chute under a non-operating screening unit, from a step ladder. Weather conditions were damp, light rain. Employee slipped from ladder step while reaching for a part, falling approx. 6-7 ft, landing on a conv. tailpulley guard on his left side, resulting in a mild concussion and 4 broken ribs.
HE TURNED HIS RIGHT ANKLE OVER WHILE WALKING TO THE SERVICE TRUCK TO GET A WRENCH. RESULTED IN ASPRAINED ANKLE.
MELTED SNOW HAD FROZEN OVERNIGHT CAUSING HAZARDOUS CONDITIONS. EE SLIPPED ON THE ICE ADN TWISTED HIS ANKLE CAUSING A POSSIBLE SPRAIN OR BREAK. AWAITING CONFIRMATION FROM THE HOSPITAL(CRACKED B ONE IN ANKLE) .
THERE WAS ICE ON THE PARKING LOT. HE STEPPED OUTOF HIS TRUCK AND SLIPPED AND LANDED ON HIS THERMOS BOTTLE.
