EE was under the hopper shoveling and a piece of rock or ice (uncertain as to exact substance) fell on EE's lower back and caused disc fractures in their back.

Fall from ladders

Maintenance employee was working on an aggregate discharge chute under a non-operating screening unit, from a step ladder. Weather conditions were damp, light rain. Employee slipped from ladder step while reaching for a part, falling approx. 6-7 ft, landing on a conv. tailpulley guard on his left side, resulting in a mild concussion and 4 broken ribs.

CONTAINMENT POND FOR SILT BROKE OPEN THE POND WALL CAUSING WATER & SILT TO FLOW INTO THE PROCESSING PLANT AREA. WATER FLOWED INTO THE ELECTRICAL ROOM CAUSING MAIN POWER LINE FUSES TO TRIP.

EE INFORMED ME THAT HE TWISTED HIS LEFT KNEE WHILE WORKING UNDER A CRUSHER (PC2). AT THAT TIME EE SAID THAT HE WOULD BE OK.

THERE WAS ICE ON THE PARKING LOT. HE STEPPED OUTOF HIS TRUCK AND SLIPPED AND LANDED ON HIS THERMOS BOTTLE.
