While cleaning slurry from and empty thickener tank, the employee's footing was compromised. The employee fell, twisting his right knee. Soreness and mobility issues were present in the right knee immediately.

Hole in bottom of surge tank was identified at begining of shift with a JSA as a hazard and was to be covered. Employee did not cover hole and later as he was reaching for a tag line that was connected to a pipe, he stepped into the hole that was supposed to be covered, and dislocated his right shoulder.

EE WAS FILLING THE COFFIN WITH WAER AT THE SX. HE NEEDED TO CHECK THE LEVEL SO HE TOOK A SHORT-CUT BY STANDING ON THE LOADED ORGANIE PIPE INTEAD OF THE STEPS DESIGNED FOR VIEWING PUTPOSES. HE SLIPPED OFF THE PIPE BECAUSE HE ORGANIC. SOLUTION HIS SHOES. HE FELL STRIKING HIS CHEST ON A 1 1/2" VALVE AND JAMMED HIS TWO MIDDLE FINGERS ATTEMPTING TO BREAK HIS FALL ON THE CEMENT. BROKEHIS
EE WAS GOING TO PUMP "ELECTOLYTE CRUD" & AS HE WALKED TOWARD THE SETTLER HE TRIPPED & GRABBED THE WAND CAUSING IT TO COME UP OUT OF THE SETTLER & SPLASHED HIM IN THE FACE WITH ELECTROLYTE. HE WAS WEARING SAFETY GLASSES BUT THE SOLUTION RAN DOWN UNDER THEM. EE FLUSHED HIS EYES & WAS TAKEN TO ER FOR IRRIGATION. AFTER REFERRAL TO OPTHOMOLIGIST EE WAS GIVEN 1 DAY NO WORK & PRES EYE DRO
