Showing all 8Employee was greasing equipment at start of shift about daylight. EE moved the grease bucket and stepped backwards off a 1-2 foot drop twisting their right knee and right ankle.
Employee sprained left ankle while walking on uneven terrain. The employee reported a prior history of a rupture of the achilles tendon in the same ankle so it is suspected that they may have re-injured it. It is also suspected that they may have an ongoing infection from the previous injury within the achilles tendon.
Employee was moving a saw blade, tripped and fell and sprained wrist.
The EE was grabbing a wooden pallet from the stack that it was in, when EE tripped over the forks of a fork lift. The forks were lowered and resting on the ground. The miner lost footing, falling to the ground and tried to catch themselves with a hand, thus hurting a wrist. No one witnessed the fall only seeing EE getting up off the ground.
EE was stepping across an empty pallet, when a board broke and his foot fell through the pallet, twisting his right knee.
Employee alleger he was getting out of the 95 xt case skidstear and his foot got stuck in the mast as he was exiting the machine to fuel it. It caused him to spin around and strike his back on the front tire of the machine.
EE ALLEGES HE WAS AIR BAGGING A SAW CUT AND HE SLIPPED IN THE SAW SLUG, GRABBED A ROCK TO CATCHHIMSELF AND CUT HIS HAND ON THE ROCK.
