EE was bleeding excessively from a preexisting wound on left ankle, due to the blood loss, ambulance was called. EE was transported, released with care instructions for a Venous Skin Ulcer. EE stated to supervisor that nothing was dropped, smashed or hit the ankle to cause the bleeding, it was an old skin injury that was not work related

EE had staged 2 slabs to saw, they were not properly square. EE took a crowbar, placed it under the top slab, while holding the bar EE used a crane to hook under the crowbar to lift the slab, when the corner of the slab broke the strap and crowbar flipped up striking EE in the nose.

Employee was splitting rock when a piece fell on the down button while hand was pulling the stone away pinching hand between the plate and the stone. Employee did not report incident until 3:15pm end of shift. Taken to hospital on 7.22 am for exam and drug screen was placed on modified duty same day.

Employee was pushing stone into the splitter and a piece of stone bounced back and pinched EE's 5th digit of the left hand in between two pieces of stone. The injury was classified as a contusion and EE was released back to full duty.

While performing normal job duties, employee experienced a seizure. EE's supervisor called 911 & the seizure stopped, EE was awake & alert upon ambulance arrival & was transported to a hospital for treatment. EE has no history of seizures & no cause is known. EE was released from the hospital & is restricted from work per EE's physician pending further testing.

Employee was operating the splitter. Employee was pulling a slab toward themselves that was underneath the splitter head. Employee hit the button that lowers the head and left hand was smashed between the the slab and the splitter chisels.

Employee was turning a cutting table by pushing with one arm and pulling with the other and strained the trapezius muscle in upper back. Employee was taken to a hospital for treatment. They performed an exam as well as a post accident drug screening. Employee was placed on modified duty.

Employee was opening a sliding door on the polishing machine by pushing with one arm and pulling with the other. EE strained a muscle in EE's upper left back under EE's scapula. EE was taken to the local hospital for examination, given post accident drug and alcohol screening and released to modified duty.

Employee was reaching across the table and a rock fell onto the back of EE's left lower leg resulting in a contusion. EE was taken to Occupational Medicine for treatment, advised to elevate the leg for the remainder of the day and return to full duty the following day.

Employee was moving a pneumatic table and when the air had not fully released employee forcefully pushed it and employee twisted their back when it moved.

Employee had EE's hand on a piece of stone as EE mechanically advanced the stone forward in the splitter. EE's hand was placed in the wrong position and EE smashed it between the guide and the stone. EE fractured the tuft of EE's index finger. EE has been working light duty with no use of the upper left extremity until EE's next follow up appointment on 7/2/2019 at 4:00 pm.

Employee sprained left ankle while walking on uneven terrain. The employee reported a prior history of a rupture of the achilles tendon in the same ankle so it is suspected that they may have re-injured it. It is also suspected that they may have an ongoing infection from the previous injury within the achilles tendon.

Employee stepped on a rock and rolled EE's ankle whenever EE climbed off of the feed table that EE was sitting on while performing a repair on the splitter gauge. Poor foot placement was to blame. EE was diagnosed with an ankle sprain and is currently performing modified duty.

Employee was assisting another employee remove a guard off the in-feed table on a rock splitter table to make a repair. While removing the guard it slipped from their hands and fell onto the foot of one of them.

When a large piece of stone jammed the conveyor belt guard, EE was attempting to pry it loose with a pry bar. When EE pulled downward on pry bar EE smashed a finger between the bar & the conveyor belt. EE was taken to hospital for treatment, given a urinalysis, the laceration was cleaned & stitched closed. EE was released to full duty with no restrictions.

EE stepped onto a 3"" water hose and rolled right ankle. EE was taken by personal vehicle to Occupational Medicine for treatment. They examined EE and diagnosed EE with a sprain.

Employee was preparing to oil equipment when EE turned and felt EE's back ""lock up"" and felt a sharp pain in EE's mid back. There were no safety violations. EE was taken to occupational medicine at SGCMH for treatment. EE was evaluated and determined to have a strain of the thoracic region. EE was ordered modified duty, with a follow up appointment in 1 week.

Employee has epilepsy, was stacking rock and fell on their back while having a seizure. Was taken to local hospital and released with no injuries, released to full duty 7/13/2018.

The IE was moving an L shaped piece of cut stone when part of the stone broke loose and fell towards the floor striking ee on the foot just above the safety toe protective area. The piece of stone struck only one foot.

The EE was grabbing a wooden pallet from the stack that it was in, when EE tripped over the forks of a fork lift. The forks were lowered and resting on the ground. The miner lost footing, falling to the ground and tried to catch themselves with a hand, thus hurting a wrist. No one witnessed the fall only seeing EE getting up off the ground.

The individual was standing at EE's work station when a piece of stationary stone broke apart and struck the top of EE's foot.

INDIVIDUAL WAS SPLITTING MATERIAL ON HYDRAULIC SPLITTER AND HAD POOR HAND PLACEMENT. THE STONE ROLLED AND EE'S LEFT THUMB WAS CAUGHT BETWEEN THE STONE AND THE CHISELS ON THE SPLITTER.

Employee was stacking product on a pallet. He was trying to force a piece of stone in between two others and the piece suddenly went between them. When the stone slipped in between them it caught his right ring finger pinching it at the nail. Employee didn't notify management until the next day and we took to the clinic.

Employee was getting ready to hone a piece of stone. He realized his grinder was unplugged. He then plugged the grinder in and the paddle switch was stuck. This led to the grinder taking off and catching the employee on the left arm causing 2 small lacerations.

The employee was ending his shift by wrapping the cord of a 4 1/2"" sander to prepare to put it away and he did not unplug the cord first. He accidentally hit the trigger and engaged the sander and it flopped over and the edge of the sandpaper lacerated his right thumb.

The employee was standing in front of a rock that was sitting on a 4x4 that was going to be placed on a pallet. To make it mobile, an employee was in the skid steer and attempted to get out of the machine without the parking brake in place. The machine moved, tipping the rock over on the injured employee's foot.

The contracted employee was installing 4"" PUC pipe in the splitter plant overhead from a man lift approx 12 ft above floor and 4-5 ft from ceiling. Throughout the installation procedure PUC glue and cleaner was used. All garage doors open in the building. He passed out, had a seizure, and transported to hospital by ambulance.

EE HAD REMOVED GOOD ROCK FROM SAW BAY, LEAVING END PIECES STANDING. HE THEN REMOVED ANOTHER STACK OF GOOD ROCK, HITTING THE SCRAP ROCK CAUSING A DOMINO EFFECT WHILE STANDING AT THE END OF THE LAST ROCK AND IT FELL OVER STRIKING HIS LEFT FOOT. THE CRANE IS CONTROLLED REMOTELY BY EE. HE LACERATED THE TOP OF HIS FOOT WITH DEEP CONTUSIONS.

THE WE IGHT OF THE BLADE TIPPED THE 4X4 UP & THE BLADE ROLLED ONTO THE TOP OF VICTIM'S FOOT JUST ABOVE HIS STEEL TOE.

A SLEDGE HAMMER WAS BEING USED TO CONSTRUCT A NEW PARTITION WALL IN THE SHOP. THE HAMMER SLIPPED OUT OF THE EE'S GLOVED HANDS AND WAS FLUNG IN A PATH TO STRIKE EE BETWEEN THE EYES. HE SLUNG HI S HEAD TO TRY TO AVOID THE IMPACT AND HIS HARD HAT CAME OFF AND THE HAMMER STRUCK HIM IN THE SIDE OF THE HEAD.

THE EE DOESN'T KNOW WHEN OR HOW HE HURT HIS BACK AT WORK, HE IS ASSUMING HE DID. HE FELT FINE AFTER WORK, WENT TO BED AND WOKE UP THE NEXT DAY WITH BACK PAIN.

EE WAS TERMINATED ON JAN. 4, 2000. HE THEN IMPLIED HE HAD A HERNIA WHICH HE HAD RECEIVED WHILE WORKING FOR OUR COMPANY. AT NO TIME DID EE REPORT ANY INJURY TO SUPERVISOR OR ANYONE AT OUR MAIN OFFICE. CONSEQUENTLY, WE WERE UNAWARE UNTIL AFTER HE WAS TERMINATED THAT HE CLAIMED TO HAVE BEEN INJURED ON THE JOB.
