Installing head liners in the ball mill, employee lost footing while hooking liner to the hoist and strained his upper back. Employee was placed on restricted duty for 2 days.

ON 2ND VISIT TO DOCTORS 11-9-05 CHANGED TO REPORTABLE DUE TO MEDICATION PRESCRIBED.
Employee was stepping off dozer when he stepped on a rock and fell backwards. He caught himself with his hands to break his fall.
EE was fueling truck- upon returning to lube truck he stepped into mud/ rocks and felt ankle roll over. Diagnosis- sprained ankle- restricted duty- reportable.

EE THOUGHT THIS WAS AN EASIER WAY OF CARRYING OUT THE I-BEAM. WHEN EE PICKED UP THE I-BEAM TO HIS SHOULDER, HE LOST HIS BALANCE AND FELL, RESLUTING IN A RIGHT INDEX, MIDDLE AND RING FINGER FRACTURE.
WHILE CHECKING 8BC-1 TO 8BC-2 DISCHARGE; WHEN GOING DOWN THE STAIRWAY EMPLOYEE SLIPPED ON MAT 4, LANDING ON HIS TAILBONE. DUE TO MEDICATION ON DOCTOR'S VISIT OF 9/13/04, THIS HAS BECOME REPORTABLE.

EMPLOYEE WAS WALKING TO THE TRUCK WITH SAMPLES OF WHEN SHE TRIPPED OVER A METAL STAKE, HITTING LEFT ANKLE.
EMPLOYEE WAS WALKING FROM EQUIPMENT AND TRIPPED ON A PIECE OF RE-BAR, EMPLOYEE FELL DOWN (RIGHT KNEE HITTING THE GROUND).

EE WAS CLIMBING OUT OF A TRUCK, STEPPED ON LADDER, AND SLIPPED IN MUD LOST FOOTING, TWISTED BACK.
