Clt was stepping from skid loader bucket to the ground, he caught his foot in a chain twisting his ankle as he fell to the ground.
Employee was on the 3rd step of a 6' ladder greasing pulleys on conveyor. The ground was soft and uneven which caused the ladder to shift. Employee fell to the ground landing on his left hip and lower rib area of his back.
EMPLOYEE WAS ENTERING HIS TRUCK AS PART OF HIS NORMAL JOB DUTIES & FELT SEVERE PAIN IN THE BACK OF HIS RIGHT CALF.
Employee was tightening chain on the fuel tank he was hauling. The trailer was wet causing him to slip. He caught himself but wrenched/strained his left knee while doing so.
EMPLOYEE WAS WALKING ACROSS THE PLANT YARD, SLIPPED ON ICE AND FELL, CAUSING A LOWER BACK MUSCLE STRAIN.
DESCENDING LADDER FROM PLANT, HIS FEET SLIPPED. HE GRABBED LADDER RUNG WITH LEFT HAND. THE JERKING MOTION CAUSED A TEAR IN LEFT ROTATOR CUFF, DIAGNOSED ON 6/21/02.
EE WAS GRABBING FUEL NOZZLE TO FUEL UP DJB. HE WAS STANDING ON ICE, SLIPPED AND STUCK OUT LEFT ARM TO BRACE FALL. HE STRAINED LEFT SHOULDER IN FALL.
