A miner made the decision to climb onto a conveyor without fall protection to assist getting a drive belt off and fell 8 1/2 feet breaking right leg.

The employee had exited the scale house and walked to the eastside to talk to a customer. While returning to the scale house, the employee tripped over a piece of broken concrete that had been buried in the sand. The employee landed on the concrete pad hurting his wrist and knee

Clt was stepping from skid loader bucket to the ground, he caught his foot in a chain twisting his ankle as he fell to the ground.

As dozer crested the top of travel way ramp the center of gravity pulled the front of the dozer down. Employee was thrown forward into the cage of the dozer from the seatbelt not being buckled correctly.

EMPLOYEE WAS STANDING AT SUPERVISOR'S PICKUP TALKING. WHEN FINISHED SUPERVISOR SHUT PICKUP DOOR SMASHING EE'S RIGHT THUMB IN DOOR.

THE CREW HAD BEEN CLEANING THE GENERATOR THAT SUPPLIES THE PLANT; WHEN THEY STARTED IT, IT SHORTED OUT CAUSING SMALL BLOWUP. EMPLOYEE WAS STANDING IN BACK END OF GENERATOR TRAILER WHEN THIS O CCURRED, AND IT CAUSED HIM TO LOSE HIS BALANCE AND FALL OUT OF TRAILER, BREAKING LEFT ANKLE.
