IP was climbing down a ladder, grabbed a swing gate for support, the swing gate broke loose and the IP lost their grip and fell less than 3 feet. The fall resulted in a sprain to the right foot and ankle.
IP was directing vehicle traffic and tripped on a concrete ledge resulting in a sprained ankle.

Employee noticed a sump pump was not working. When they walked out to the pump they slipped, twisted ankle and landed hard on back. Employee went to ER for eval. and xrays. No broken bones. On 09/29/20 employee complained that ankle was still hurting. We took them to Ortho & They found a break in the femur above the ankle, put them in a boot.
An operator was descending down a set of stairs located at the back of the warehouse. The operator missed/ slipped on the last two steps resulting in a fall. The operator claimed to have no injuries but was sent to the local medical facility for a proper diagnosis. The employee indicated that they were using the handrail at the time of the incident & there were no witnesses.
The employee was walking on sloped concrete near plant 32 truck loading when employee slipped and fell. There was a thin layer of slurry type material that has washed there from the 1" of rain we got overnight. As employee fell employee tried to catch self with right hand. Employee's right ring finger was bent during the fall and resulted in a fractured finger.
Miner dismounted dozer to go to the restroom, when mounting dozer miner's foot slipped off the step causing the miner to catch self with the left arm.
Employee pulled up in a back hoe to address EE's union rep. When EE went to climb over a one foot wall EE's foot slipped and EE fell, straining EE's shoulder.
An operator was walking to the break room. EE stepped in a hole (grass was grown over the hole) and EE rolled ankle. It was diagnosed as a fracture.
EE twisted knee when EE slipped and tried to catch self.
EE was leaving the Operations Office (not during EE's work shift). EE left the facility and turned left to follow the sidewalk. EE then saw a barricade on the sidewalk but then slipped and landed on EE's left hip. EE went to the Main Street Family Urgent Care where EE was examined and X rayed. EE also was tested for drugs and alcohol (negative test results).
Employee was running to turn off vacuum truck when ee felt a pop in left foot.

A bulk loader fell from a tanker truck. Their harness arrested the fall and the swing fall carried them into the structural steel in the loading alley. No injury from the fall; Contusions to their legs from the impact of the swing fall.
EMPLOYEE STEPPED DOWN FROM PLATFORM & TWISTED ANKLE.
Employee was attempting to remove ceiling tiles from a 8' ceiling in mining office. Employee did not have proper ladder for project and was working out of an office chair when the accident occurred. Employee twisted a knee and is awaiting an MRI on 02/13/2017. The x-ray's showed possible torn ligaments/tendons to be determined after MRI.
Employee was getting into the Fuel Truck to exit the mine and employee's foot slipped on the step causing employee to fall backwards landing on a rock with employee's lower back
Tripped climbing onto bulk truck. Fell striking EE's right knee on ladder of truck resulting in laceration & stitches
Supervisor was descending stairs and hung left heel on the last step causing supervisor to fall.
Employee was descending the stairs coming out of plant 3 Operator's Station and lost footing and slipped. Reported the event to supervisor and refused medical treatment. Returned the following shift with swelling and bruising in lower back.
Miner was walking across an area of snow covered natural earth when he states he slipped and twisted his right knee. Miner was taken to County ER where a licensed physician performed physical assessment. No x-rays taken, diagnosis was knee sprain. Leg Splint to immobilize was applied, 800 mg of ibuprofen prescribed, Miner returned to work with restrictions.
The Drill Op. used step ladder to access oil reservoir at top of engine block. After completing task, operator descending diesel tank platform onto step ladder and fell to ground. Operator struck his left side/ribs on corner of fuel tank platform and also struck his head and fell to the ground causing bruising to ribs and head injury resulting in concussion.
Bucket elevator was being replaced and pulled by steel cable. Buckets dug into ground putting tension on eyelet of steel cable. Cable slipped open, wrapped around employee's shoe, knocked employee onto ground on right hip.
Operator was walking the catwalk near the crusher and, on his way back from the crusher area, he slipped/fell onto the catwalk. There small laceration to the left knee with moderate swelling 24 hours later. Employee states that he wasn't sure why/how the fall was caused, but believed it may have been due to cold/icy conditions outside.
Employee was exiting pickup truck to apply wheel chock, which was secured to the bed of the truck with a piece of 1/4" cotton blend rope. Upon completing placement of wheel chock, employee turned to walk away and caught right foot on rope, causing him to fall forward; resulting in fractures to right and left wrists.
Employee slipped while standing on 3 foot high wall, fell to ground, slight cut to hand, bruise to left leg.
Employee was standing on a wind box attaching and applying pressure to a bolt to a lid on top of the mill. EE slipped and fell down apx 5-1/2 feet and hit is right hand on equipment guard, while falling cutting his hand and landing on his left side and back.
EE was making a delivery to the truck shop. When he stepped down from the cab of the truck, his foot turned under him twisting his ankle.

Employee took one step down stairs inside Plant 3, woke up on the stair landing, does not remember falling or how he got to landing. Had small contusion on left elbow. Was treated at local ER for elbow and tests to determine why he fainted.
Employee walking down steps at end of plant 2, stepped onto ground, twisted right ankle. Treated at local ER, mild strain.
Descending ladder from screens to roof of pelletizing plant, stepped on door left in wrong place (partially in ice/snow). Lost balance, fell forward on both knees and chest, bruised right knee. Reported accident to management on March 19, 2008. Received medical treatment by doctor on 4/9/2008. Continued normal job duties.

Reported incident to Geneva, NY field office and advised by Bill Korbel to submit accident report. Driver treated at emergency room for possible concussion and abrasions.

Employee was bagging super sacks (2000# bags). When he released the holding straps, the filled bag began to lean and roll towards him. He attempted to manually hold the bag in place, it pushed him to the concrete floor, straining his lower back.
