The employee was working on the platform level of 471-BF-07. EE was raising cages by a rope to the upper section of the dust collector. EE looked up to communicate with the another employee. EE stepped backwards while continuing to look up and tripped over an empty box. When the employee braced EE's fall with EE's hands causing the injury to EE's right wrist.

The employee was walking down the stairs at 1st bank silos when EE slipped on a piece of clinker causing EE to land EE's right foot awkwardly on the next step. On 03/05/2026 EE was issued light duty restrictions.

The employee was descending a stairway at the fly ash stamler. EE missed the last step and landed awkwardly. The next day the employee requested a medical evaluation which identified a lumbar (back) strain.

While stepping onto a parking pad outside of the electrical shop building, the employee slipped and fell to the ground. Employee landed on torso.

While walking up the stairs to the 3rd bank silo loadout. The employee was carrying two plastic bags in left hand and had backpack over right shoulder while on the way to the control room to start shift. The employee tripped on a stair tread and reached right arm out to brace the fall.

The employee was dismounting a railcar and felt a pop in their left leg. EE continued walking along the railroad tracks and discomfort increased. On 07/23/2024 the employee was issued a work restriction and physical therapy.

While walking from the parking lot to the building to begin working, the employee tripped over an uneven seam in the concrete parking lot. This caused EE to lose balance and fall to the ground landing on their hands and knees.

At approximately 4:oopm the employee was installing guards on the 471-DB-04 pan conveyor. While carrying a section of guard to the tail end for installation, the employee slipped on mud and fell to the ground with the guard section landing on EE's leg and back. The employee was issued a work restriction and prescription medication.

While walking through the kiln/cooler area on 10/09/2023, the employee slipped on dust that had settled in the roadway causing EE's elbow to hit the pavement. On 10/10/2023 the employee requested a medical evaluation. On 10/24/2023 the employee had a MRI to evaluate the injury and that MRI revealed a fracture of bone and partial tear of tendon.

The employee was working on a kiln hydraulic pumps repair, and when EE was complete EE stood up and began to leave the area. When EE began walking, EE's boot caught on a hose that is attached to the equipment causing EE's to trip and hit EE's knee on the ground.

While going up the stairs on the second level of finish mill 2 to clear an airslide; the employee missed a step and tripped causing employee to land improperly on the next step.

The employee was standing on a step attached to the tractor tote to check the level of material. The step broke from under the employee causing employee to hit their back on an adjacent containment wall. Employee was under the care of their dr. for an unrelated issue at the time. On Jan. 23, 2020 the company was notified that the injury was not caused by the unrelated issue.

On 04/28/19 the employee twisted EE's right foot while walking into the bypass dust collector compressor room. The employee initially refused medical treatment. On 05/10/19 the employee was referred to an orthopedist after an x-ray showed a fracture in one of the bones in the foot.

An employee was walking along the rail tracks where repairs were being made on an earlier shift, they tripped and hit a knee on railroad track that was set aside to replace existing track. The injury was reported on 12/21/18, the employee refused medical treatment at the time. on 12/26/18 the employee requested medical treatment and was issued a light duty designation.

A customer truck (over-the-road truck driver) was finished getting tractor trailer loaded, customer walked up to the hatch closing station and closed the hatch. Customer then walked down the stairs from the hatch closing station and to the cab of the truck. As customer walked to the cab of the truck; customer stepped in a hole and injured back.

The employee was walking into the electric shop and tripped on the step. Employee reached out to catch themselves and cut a hand on an adjacent sink.

Went to cross over a discharge pipe (PVC) and did not realize that a patch of ice had formed on top of the pipe. Placed foot onto the pipe to step over it and slipped. The employee injured their wrist when they braced themselves upon landing.

Material on a pallet that the employee was walking on slid off of the pallet. This caused the employee to fall causing a contusion on a finger and a sprain in back.

While walking from 471-DB-04 to the electrical room; the employee tripped and injured ankle.

The employee was going to the pumps in the quarry to check their status. EE stepped over a pipe and observed a snake in the shadow of the pipe. As EE turned to get away from the snake, EE caught boot on the pipe and hit knee and hand on the ground.

The employee was walking to a loader to begin operation. The ground was still wet from rain where the loader was parked. As the employee approached the loader (approximately 3' from the loader) EE slipped. EE was diagnosed with a contusion and an abrasion on EE's right arm.

The employee was working in a secondary containment for grinding aid; behind 2nd Bank Silos. The employee slid awkwardly while standing in the dry containment area and injuring EE's left foot/ankle.

While walking along the cooler area of the plant, the employee sprained his right knee. This resulted in the employee being placed on restricted duty.

While exiting a Caterpillar 980M front loader the employee slipped while exiting and bumped his left elbow on the loader causing a laceration that required sutures.

The employee slipped and sprained his left knee while working on the BC-05 conveyor from the associated catwalk.

The employee stepped off of a platform for the north roller at the vertical raw mill and twisted (sprained) knee.

While walking out of the administration building; descending the north stairs the employee felt a sudden sharp pain in his right knee. He did not know the cause of the pain; there was no loss of balance or impact. The employee is on light duty as a result of the injury.

The employee was cleaning out an airslide when the cement free flowed out and the employee tried to climb down a ladder. Due to the free flowing cement there was no visibility. The employee fell off the ladder approximately 7-8 feet up to a catwalk below. The employee sustained an abrasion to the right bicep.

Employee slipped and fell onto a slab of concrete. The employee landed on his right side fracturing two ribs.

Employee was cleaning the feed end of two finish mill using a shovel and a wheel barrow. The employee entered the east door pushing the empty wheel barrow and set it down. The employee began walking on the clear walkway to retrieve a shovel when his left foot got caught on the grating causing him to trip. The employee sustained a sprain and minor contusion to the left knee.

EE was walking through the plant and tripped on some rocks and fell. When the employee fell, he struck his head on the ground causing a laceration to the left side of his head.

EE was walking near building and stepped into a puddle of water and slipped and fell. This cause a sprain to his right ankle.

EE was working on and elevated platform removing duct work. When the duct work was removed, a small hole approximately 2'X 2' remained. The employee stepped into the hole, causing him to fall against the adjacent hand rail. The accident resulted in two fractured ribs.

EE was opening the rear gate of a roll off box on a delivery truck. The employee slipped and fell backwards onto his left wrist, causing a fracture.

EE was working on repairing a safety sign when he lost his footing and twisted (sprained) his left ankle

Employee was refueling a D65 Komatsu Loader when he slipped off of the tracks and landed on the ground. Employee had no broken bones, but complains of complete body soreness.

EE was dismounting a Cat 773 haul truck when his right hand slipped off the handrail. EE swung backwards and jumped to the ground, spraining his right ankle.

Equipment operator stated that he slipped while getting into the cab of the D-11 Dozer. Upon slipping, he grabbed the ladder and twisted his left wrist. Diagnosis is a sprained wrist.

While climbing into cab of dozer, employee slipped and grabbed ladder spraining his wrist. Subsequent evaluation 12/6/07 indicated that employee had sustained a torn ligament in his left wrist. Was on lite duty from this time. Surgury was on 1/7/08 at which point this becomes a lost time.

EE was re-stacking bags of cement. Turning while lifting, his right foot slipped and he felt pain in his lower right leg.

Employee was walking between two rows of bricks and tripped on a metal band that was sticking out from under a pallet. The employee caught himself on his hands as he fell thus injuring his wrist.

EE was walking down a flight of stairs and slipped on the last step and fell.

While unloading vacuum hose from a flat bed truck, employee placed a piece of hose on the ground. When he began to remove another piece of hose from the truck the employee fell over the piece that was on the ground.

Employee was stepping off of a ladder on a front end loader and stepped on a rock and fell twisting his ankle.

Employee was climbing a ladder and lost footing, left hand went between the guard and a cover on a machine. Employee's hand came in contact with a slow moving conveyor thus lacerating three fingers on the hand.

Employee was was walking to the crane and slipped and fell catching himself on his right hand.

Injured employee was walking on a catwalk and slipped and fell injuring his right forearm

Employee was walking between the trench and fly ash silo to plumb a line for boards. Employee's right foot slipped as weight was being put on the left leg, causing him to fall on his right side. Employee hit his right elbow against the fly ash silo foundation or footer.

Employee slipped on clinkers & twisted his right knee

Employee stepped off of frame of belt and turned ankle
