Employee was walking in stockpile area, slipped on ice covered with dust falling and striking right shoulder on ground. After continued pain in shoulder EE was sent for evaluation on 1/31/2022, MRI revealed torn rotator cuff in right shoulder. Injury became reportable on 2/18/2022 when surgery was performed to repair torn rotator cuff in right shoulder.

Miner was found lying on ground left side of 990H Loader, was seen minutes before on railed platform at rear of Loader. Miner was transported by ambulance to hospital, responsive at time of transport, trying to talk, attempting to sit up, remains in hospital for treatment for injuries.
Supervisor was walking at ground level of kiln pier, EE slipped on unpaved surface twisting right knee. Slight tear to right ACL occurred. This accident became reportable on 05/31/2016 when employee had surgery on right knee.
Employee missed last step on ladder when exiting loader. Fell to ground landing on right elbow and shoulder, contusion of shoulder.
Employee was opening hatch on top of rail car, car moved unexpectedly and employee slipped off side. Employee was attached by harness to a retractable lanyard, employee was swung against car fracturing bone in left wrist.
Employee was walking down steps on west side of cooler, he tripped on step and felt pain in his right knee, diagnosed with knee strain. Employee was able to fully perform job functions and returned to full duty. Pain continued in knee, sent for MRI. Employee was sent for surgery December 30, 2010, arthritis aggravated by trip on the step.
Employee was walking on roadway, stepped off roadway and twisted right knee. Employee was sent for evaluation, he was able to fully perform his job until 08/24/2010 when pain increased in knee and he was sent for MRI. Diagnosis was torn meniscus with no work restrictions placed on employee. This injury was deemed questionable and has now been deemed work related and reportable.
Employee was on the second step of a step platform 16 inches off the ground, he turned to lift down a section of 3 inch pipe that had been cut away from the main line. As he turned his right foot stepped on his left foot causing him to slip off the step, his left foot caught under the step twisting his left leg. He felt pain in his left shin, transported to hospital.
Employee was walking through Packhouse Warehouse to inspect a sump pump, he stepped in a cracked portion of concrete floor and twisted his right ankle. The broken concrete was 10 inches by 12 inches and 1 inch deep. Employee sustained sprained right ankle.
Employee was walking through area at new kiln, when he twisted his left ankle and foot. Medical evaluation revealed a fractured bone in foot. A cast was applied and work restrictions mandated. The employee is a manager and is able to perform his job duties within the restrictions.

Employee who is a supervisor went to company doctor who released him for work, follow up showed torn medial meniscas in right knee. EE WAS CLIMBING INTO THE QUARRY HAUL TRUCK WHEN HIS FOOT SLIPPED OFF THE LADDER, DUE TO MUDDY CONDITIONS, AND HE INJURED HIS ANKLE.
EE WAS STEPPING OVER PIPE ON TOP OF TAMKER AT BULK LOADING STATION TO DO A COLOR CHECK WHEN HE SLIPPED ON THE PIPE & TWISTED HIS KNEE.

EE OVER TREAD HIS RIGHT ANKLE BY PARTIALLY STEPPING ON A 2" EDGE THAT HE DIDN'T SEE.
EMPLOYEE LOS THIS GRIP CLIMBING THE LADDER INTO THE TRUCK AND FELL TO THE GROUND ON HIS FEET. THEN TWISTED INTO THE TIRE OF TRUCK.
EE WAS CARRYING PARTS FROM #1 KILN COOLER AREA AND STEPPED IN A SMALL HOLE AND OVER TREADED HIS LEFT ANKLE.
SUPERVISOR WAS GETTING OUT OF PICKUP TRUCK AND SLIPPED ON ICE THAT WAS SNOW COVERED.
