Employees knee buckled while descending the stairs and employee fell and twisted their knee.

Employee was removing debris from the Blunger. As EE was removing debris, EE's foot slipped. EE put left arm up to catch self and EE's forearm hit the wear ring on the Blunger causing a laceration requiring six stitches.

Employee was in front of the steps at B tank manifold when EE tripped on an uneven surface. EE stuck EE's arm out to break EE's fall and jarred EE's arm.

3 mechanics were working on a screw and motor. when the last bolt was removed, the screw twisted around and bumped the mechanic and knocked EE down.

EE was walking down some steps and EE's feet slipped out from under and EE fell on right shoulder.

Employee was exiting warehouse C thru the rear door. Employee slipped and fell on the wet concrete floor that had a small section of algae on the floor. They landed on their left shoulder. Employee got up and continued working. They requested to go to doctor on 3/6/19

EE was descending the calcine silo stairs carrying post mill samples (3 samples weighing approximately 6lbs each), EE turned to stepped down from the landing to the next step. While shifting EE's weight onto one leg, EE felt a sharp pain in EE's left hip followed by a burning sensation down EE's left leg

Employee was moving pump hose when EE stepped into a hole and complained of possible foot pain. Uneven ground conditions contributed to this injury.

Employee turned to come down 3 steps and lost balance and fell to concrete floor.

Low-Boy driver was picking up a bull dozer. After climbing up onto the tracks he was in the process of trying to open the door. Before he was able to open the cab door he somehow lost his balance an fell to the ground. The driver suffered a fractured Tibia from the fall.

Miner miss-stepped while preparing to go up steps leading to loading station; causing him to fall ground level. Miner tried to break his fall extending his right arm out, on impact miner struck arm against railroad cross tie. At the time of the accident minor was treated w/first aid on sight. Accident didn't become reportable until Jan. 9 when he received treatment from physian.

2 employees were removing handrail when one repositioned hand and rail got away throwing other off 36" ramp to ground. (Asphalt)

Employee heard a loud noise and turned to go down stairs and twisted his ankle.

Employee was repairing a water ring. A piece of the floor grating was removed to access the area. The employee stepped back and fell in the opening. The employee recieved a laceration to his right elbow.

Employye was washing up inside a railcar and slipped and fell on side.

Employee was removing coupling from pipe and product began coming out of pipe and employee stepped to get out of area and strained back.

EE was looking under railcar for chocks and tripped over pothole.

Employee was walking toward a valve. He went to step onto a step to enter a crosswalk when his foot slipped out from under him. Employee's left leg was broken.

ENTERED PLANT OFFICE AREA THEN RETURNED TO PERSONAL TRUCK TO RETRIEVE SOMETHING. UPON RETURN TO OFFICE AREA TRIPPED ON PAVED CURBING & FELL ON RIGHT HAND. 2 BONES IN RIGHT HAND FRACTURED.

AT END OF SHIFT EE DISMOUNTED HOPPER RAILCAR & BEGAN WALKING ACROSS CROSSING AREA BETWEEN TRACKS & STEPS AT ENTRANCE TO CALCINE BAG SHACK WHEN HE SLIPPED & FELL, BRUISING LEFT ELBOW. AREA HAD JUST BEEN WASHED OFF & WAS CLEAN, BUT WET & SLIPPERY.

Employee slipped/tripped in white clay building during power failure. Employee was going to restart a water booster pump.

Employee had mud on boots and slipped on step causing him to fall on right shoulder. The employee has been at work everyday and worked at full duty with no restrictions until he had out patient surgery today to repair a torn rotator cuff.

Operator was coming down ladder off an off-the-road truck when he apparently lost his footing and sprained his left ankle. Continued to work until about 9:00 pm when it started hurting and he went by the E.R. where ace bandage was applied. He is to wear wrap 48 hours, apply ice, elevation and use crutches until 7/29/05.

GETTING DOWN OFF EARTHMOVING TRACTOR/LAND LEVELER & LOST HIS FOOTING. FELL & WHILE REACHING FOR HAND RAIL HE CAUGHT HIS FINGER, BENDING & BREAKING IT. HE THEN FELL ON HIS RIBS BRUISING THEM. HE WAS TAKEN TO ER, THEN TO SURGERY TO REPAIR HIS LITTLE FINGER ON HIS LEFT HAND. OUT OF WORK UNTIL RELEASED BY PHYSICIAN.

Employee entered C-17 pumphouse and tripped over a drain line. He landed on both knees and both hands.

Employee slipped and fell when he stepped in liquid clay slurry deposited on concrete while he was getting a clay slurry sample from a storage tank.

Employee was descending a railroad slurry car ladder when his foot slipped and he fell to the ground. He sustained a sprained left ankle.

EMPLOYEE WAS STEPPING FROM A SLURRY RAILCAR LADDER TO THE GROUND WHEN HE TRIPPED AND BROKE HIS LEFT WRIST WHILE TRYING TO CATCH HIS FALL.

GETTING OUT OF TRUCK, SLIPPED ON CLAY, HIT BOTTOM RAIL STEP OF TRUCK WITH LEFT LEG CAUSING LACERATIONS REQUIRING SUTURES.

EE WAS TRYING TO OPEN A VALVE THAT WAS FITTED WITH A "T" HANDLE. THE HANDLE BROKE AND THE EE STUMBLED BACK HITTING HIS HEAD ON A HEAT EXCHANGER PLATE CAUSING A LACERATION TO HIS HEAD (10 STITC HES REQUIRED). HE ALSO CHIPPED A TOOTH WHEN THE "T" HANDLE HIT HIM IN THE MOUTH.

EE TRIPPED OVER A PRUTUDING FLOOR BOLT TRIPPED AND JAMMED HIS RIGHT HIP WHEN HE CAUGHT HIMSELF.

EMPLOYEE STEPPED OFF BACK OF TRUCK ONTO BROKEN CEMENT; TWISTED RIGHT ANKLE; RESULTING IN SPRAIN/BRUISE TO ANKLE.

UPON APPROACH TO STAIRWELL, EE SLIPPED DOWN AND LACERATED FINGERS ON GRATING (FLOOR). LACERATION REQUIRED SUTURES. MINOR SCRAPES TO LEFT LEG & ARM.

WALKING ACROSS ROUGH CLEARED GROUND AND TRIPPED OVER A ROOT AND FELL ON SIDE. WHEN EMPLOYEE STOOD UP, LOWER BACK WAS HURTING SEVERELY.

Employee stepped off a forklift and felt a pain in his left knee. The accident was not initially reportable to MSHA. He continued to work unrestricted until 02/17/05 when surgery was preformed to correct the pain problem.

EE WAS ATTEMPTING TO RELEASE BRAKE ON HOPPER CAR. CHAIN WOULD NOT RELEASE EE APPLIED MORE PRESSURE, FOOT SLIPPED BRUISING INSTEP OF LEFT FOOT WET WEATHER CONDITION.

EMPLOYEE STEPPED OFF BOTTOM STEP ONTO CONCRETE FLOOR; LEFT ANKLE TURNED CAUSING EMPLOYEE TO FALL RESULTING IN SPRAIN TO LEFT ANKLE.

MAINTENANCE EE WAS STEPPING BETWEEN TWO CONCRETE MOUNTED PUMPS AND HIT HIS HARD HAT ON A BEAM CAUSING HIM TO FALL.

WASHING UP CLAY SLURRY ON CONCRETE BETWEEN CURBING/DIKES AND MISPLACE FOOT SUPPORT CAUSING FALL TO LEFT SIDE, WHICH RESULTED IN A LACERATION REQUIRING TWO SUTURES ON LEFT ELBOW.

EE SLIPPED AND FELL OFF STAIRS.

SLIPPED OFF LADDER INSIDE RAILCAR.

EMPLOYEE PUSHED STARTER BUTTON, BREAKER SHORTED INSIDE ELECTRICAL PANEL CAUSING PANEL BAR TO BURST OPEN CAUSING EMPLOYEE TO FALL BACKWARDS ONTO HIS RIGHT LEG ON GRAVEL SURFACE, WHICH RESULTED IN A LACERATION TO HIS RIGHT LEG NEEDING SUTURES

SLIPPED WHILE TRAVELING/ WALKING AND FELL FORWARD ONTO LEFT LEG CAUSING BACK, HIP, AND LEG SPRAIN TO LEFT SIDE.

WASHING MAKEDOWN MIXER OVERFLOW OF DRY CLAY PRODUCT OFF METAL SURFACE WITH WATER CREATING SLICK SURFACE OF CLAY AND WATER. CLAY & WATER MIXTURE DOES NOT PROPERLY DRAIN AT THIS LOCATION.

UTILITY WORKER CLEANING UP EXCESS POLYMER AND CLAY SLURRY. SLIPPED WHILE CLEANING AT WHITE CLAY BUILDING.
