Struck against stationary object

Employee was removing debris from the Blunger. As EE was removing debris, EE's foot slipped. EE put left arm up to catch self and EE's forearm hit the wear ring on the Blunger causing a laceration requiring six stitches.

Employee tripped over a raised concrete area, lost his balance, reached out to break his fall, and fell on his hand fracturing the left index finger. EE also sustained a hematoma to distal end of same index finger.

HSI driver was traveling east on Gibraltar Mine haul rd. As the driver started negotiating to make a left turn curve he lost control of his vehicle.

EE WAS AT GRADE ADJUSTING THE LOCATION OF FORKS ONT HE FORKLIFT WHEN THE FORK CAUGHT HIS THE TIP OF HIS LEFT RING FINGER.

EE WAS WORKING WITH AN EXPERIMENTAL ROD MILL AT THE DIXIE MILL. THE UNIT HAD SHUT DOWN WHEN IT BECAME CLOGGED & THE FEED WAS DISCONTINUED TO THEUNIT. WHEN THERE WAS NO FLOW, HE CHECKED THE VAL VE BETWEEN THE TWO CHAMBERS TO CHECK FLOW. HE PINCHED HIS LEFT INDEX FINGER BETWEEN THE SLIDING LOCK & THE HANDLE STOPS ON THE VALVE BODY.

HELPING TO UNLOAD A BULLDOZER IN THE RAIN, EMPLOYEE REACHED OVER AND PICKED UP A LOWBOY RAMP ANDFELT SOMETHING POP IN RIGHT SHOULDER.
