Employee was taken to Doctor where his elbow cut was closed with glue instead of suture. Employee returned to work with no restrictions.

TRIPPED ON BOLT THAT HOLDS SCALE INSPECTION COVER IN PLACE. FELL APPROX. 2' TO GROUND ON HIS LT.SIDE.
EE WAS UNLOADING A ROLL OF WIRE FROM TRUCK WHEN HE LOST HIS FOOTING INT HE MUD AND FELL ON HIS TAILBONE.
WHILE GETTING OUT OF STOCK TRUCK, EMPLOYEE MISSED A STEP AND FELL TO THE GROUND, LANDING ON HIS RIGHT HAND.
EE WAS EXITING STOCKPILE TRUCK WHEN HE CAUGHT THE TOES OF HIS LEFT BOOT ON THE BOTTOM STEP ON THE TRUCK CAB. HE THEN FELL TO THE GROUND, TWISTING HIS ANKLE THAT WAS CAUGHT IN THE STEP.
EE SLIPPED AND FELL TO FLOOR WHILE ENTERING RESTROOM AT QUARRY SCALE HOUSE. HE STRUCK HIS BACK &HEAD ON THE FLOOR.
