Employee was assisting mechanic replacing springs on a Mack Haul Truck. When all repairs were complete, employee was lowering Mack Truck with rachet type jack when the handle raised quickly and pinched employee's left index finger between the bed and the handle. Mine foreman took employee to hospital where he received x-rays and 8 stitches to left index finger.

Employee stepped off of Dozer and twisted knee which aggrevated a previous condition to his knee. Employee injury was diagnosed as chronic ACL tear & meniscal tear.
Employee raised shop overhead door manually and strained muscle in mid to lower back area.

Employee was attempting to turn the bowl on the Standard crusher at the Tertiary when one of the pull lugs on the bowl broke off. It struck the employee in the leg just below the boot line causing a fracture in the Tibula.
Employee walking across yard and took his safety glasses off to clean them while still walking and walked into a set of detachable pallet forks that were lying on the ground. Employee's left leg struck the forks in the shin area and caused a laceration that required sutures. He was able to return to work with no restrictions.
Employee working on feeder at the tunnel belt was sitting up after crawling out from under pan and struck his head on a flow paddle switch causing a minor cut to his forehead. Employee received a tetanus shot and dissolvable stitches and returned to work with no restrictions.
While pulling hose on pumper truck felt severe pain in neck. Employee did not seek medical attention until 7/16/07.
Employee was using a wire brush in a hand drill with safety glasses on and got something in his right eye. Employee was sent to eye doctor and no material was found in eye but was given prescription antibiotics with no restricted duty.
While shoveling a pile of stemming, he hit a large rock in pile & felt sharp pain in shoulder. Shoulder strain
Mechanic was pulling on prybar when bar slipped hitting him in the mouth.
Employee was exiting Mack Stock Pile truck and missed the bottom step on the fuel tank. He fell and hit his tail bone causing a bruise.
Employee was welding a plate in the Jaw feeder when a rock fell approximately 2 feet from the side of the feeder and hit or rolled on his middle finger on his left hand. Employee injury resulted in 4 sutures to his finger.
Employee was operating the Cat Skid Steer loader when he swerved off the roadway and hit a small drainage ditch to go by an oncoming Cat 980G loader, that resulted in a lower back injury. Employee received cortisone shots, steroid packs, pain pills and was put on restricted duty pending his follow up visit by the physician.
Employee was dislodging material stuck in a discharge chute when his right hand got pinched between the shaker deck and the chute. Medical treatment consisted of a rigid splint and pain pills. He was on light duty for 2 days and no follow up visits were required. His injury was a bruise to the top of his right hand.
Employee was performing maintenance at Jaw Crusher and was using a pocket knife to cut a plastic tie strap, when the knife cut through the strap and cut the top of his left hand through his glove. Employee received medical treatment consisting of 2 stitches and a tetanus shot. No restrictions.
Employee was working under the hood of the Cat 988F loader when the lower support bolt that holds the safety support cylinder sheared off allowing the hood to fall striking employee in the back. Injury is bruise on back and is currently on restricted duty.
Employee was exiting Mack twin screw haul truck and fell when his footing slipped on fuel tank. As the employee fell his left elbow was cut from hitting the rocker panel. Employee was taken to Doctor where his elbow cut was closed with glue instead of suture. Employee returned to work with no restrictions.
Employee exiting truck tripped over wood block and fell on elbow.
Employee strained back while changing out return roller on conveyor belt.
Employee was using a hammer to break rock loose from around primary crusher when the hammer scraped the back of his leg causing an abrasion in the leg calf area.
Employee was removing bearing from housing of a shaker screen deck and hit his finger with a hammer. Injury resulted in sutures and broken finger.
Employee was cleaning stuck material from hopper and 6" diameter chunk of compacted dust hit his leg causing a bruise.
Stepped off the back of a F350 Box Truck into a rut, sprained right ankle.

Employee was cleaning up under a conveyor with a water hose. When water was turned on his footing was not sufficient to hold the pressure and caused him to slip and fall. DELAYED MEDICAL TREATMENT.
While lifting a bearing assembly after it had been removed from a shaker screen, strained left arm while lowering it to deck.
Employee slipped on bank, and while trying to break fall, fell on right arm, injuring right wrist. No restricted duty initially involved.
Employee's right hand struck by cone crusher feeder plate when a welded lifting eye broke.
WHILE EE WAS STEMMING HOLES, FELT A SHARP PAIN IN SIDE. CONTINUED TO WORK. NEXT MORNING IT WORSENED. IT WAS BELIEVED THAT THIS WAS A "FLARE-UP" OF A PREVIOUS INJURY THAT OCCURED IN A MOTOR VEHICLE ACCIDENT WHILE WORKING AT ANOTHER SITE. UPON INVESTIGATION OF OUR WC CARRIER IT WAS DETERMINED ON FEB 1 TO BE A NEW INJURY.
Employee was helping finish up conveyor belt change-out. While using a utility knife, the knife slipped cutting his left index finger, requiring sutures.
STEMMING HOLES, CARRYING 5 GALLON BUCKETS OF GRAVEL AND BACK GOT A LITTLE SORE. IT BECAME WORSE OVER THE WEEKEND.
A steel bar had been inserted under a conveyor belt to hold it up while cutting a portion of a metal plate. When attempting to pull the bar out the opposite end slipped off the conveyor frame resulting in the weight of the bar pinching employee's finger tip.
Employee fell to ground while dismounting Cat 980B front end loader.
UNLEVEL GROUND.
Employee was struck by equipment as it was being lifted off a flatbed truck. Equipment was 2'x2'x30' conveyor leg (box frame type). Hook on sling disconnected causing load to shift and strike employee on right lower leg, resulting in fracture.
Stemming holes, carrying 5 gallong buckets of gravel and felt pain in right shoulder. This is a re0injury of a shoulder injury from the previous week for which he did not seek medical treatment.
Leg caught under skid steer loader being removed for repairs. Right ankle abrasion.
EE SLIPPED & FELL WHILE ASSISTING IN LOADING SHOT. IN AN ATTEMPT TO BREAK FALL, HE LANDED ON HIS RIGHT HAND & BRUISED HIS KNEE. EE HAD ON ALL PPE WHICH INCLUDES GLOVES. FIRST AID WAS ADMINISTE RED AT THE SITE THEN WENT TO OCCUPATIONAL HEALTH CENTER FOR 5 STITCHES IN RIGHT HAND.
DELIVERY TRUCK DRIVER WAS RAISING TARP BY HAND WHEN HAND BECAME CAUGHT BETWEEN CHAIN AND SPROCKET. LEFT INDEX FINGER WAS CUT OFF.
HIS HAND CAUGHT IN BIN GATE WHILE CLEARING STUCK ROCK. INITIALLY NOT THOUGHT TO BE REPORTABLE.
STRUCK AGAINST CONVEYOR FRAME WHEN BAR HE WAS USING TO DISLODGE A ROCK FROM ROLLER WAS CAUGHT IN CONVEYOR. NO MEDICAL TREATMENT UNTIL 9/8/03.
TRIPPED ON BOLT THAT HOLDS SCALE INSPECTION COVER IN PLACE. FELL APPROX. 2' TO GROUND ON HIS LT.SIDE.
EE STRUCK HAND WITH HAMMER WHILE HITTING A DRIVE SHAFT.

EE WAS UNLOADING A ROLL OF WIRE FROM TRUCK WHEN HE LOST HIS FOOTING INT HE MUD AND FELL ON HIS TAILBONE.
EE STRUCK HIS HEAD AGAINST A BOLT ON A BRACKET WHEN THE WRENCH HE WAS USING TO FREE A PTO CABLEUNDER A STOCK TRUCK SLIPPED. HE JERKED HIS HEAD TO AVOID BEING STRUCK BY THE WRENCH, BUT STRUCK HIS HEAD AGAINST THE BOLT.
