Showing all 5EE was pulling barge line on stern on barge 43, he stated the barge was muddy and he slipped, feeling something wrong in his left knee, he had no pain right away but by the end of the day he felt it hurting. Medical attention 1/28/15, MRI in Torrance Ca on 1/31/15, limits no prolonged walking/climbing, he is still working, we will update report when results from MRI are received.
EE WAS STEPPING OFF OF BREAKER. HE STEPPED ON THE TRACK AND HIS BOOT GOT STUCK IN BETWEEN TRACKS. HIS WEIGHT & MOVEMENT CAUSED HIM TO KEEP GOING DOWN ONTO HIS RIGHT LEG. HE TWISTED HIS RT. LEG AND HEARD A POP IN HIS RIGHT KNEE.
EMPLOYEE WAS PLACING LEADS OVER A CONVEYER AT SEPARATOR PLANT #2. HE STEPPED OFF A BEAM AND JUST LOST HIS FOOTING. WHEN HE FELL, HIS OUTSTRETCHED RIGHT HAND RECEIVED A LARGE LACERATION TO HIS PALM.
HE WAS PERFORMING A SECONDARY BLAST ON A ROCK (25-30 TONS). WHILE STANDING ON TOP OF THE ROCK, HE LOADED THE CHARGE AND LIT IT. WHILE STANDING UP TO EXIT THE ROCK, HE SLIPPED AND FELL OFF THE ROCK. HE LANDED ON HIS LEFT SIDE ON THE CLOSED TAILGATE OF THE PICK UP TRUCK. HE RECEIVED SEVERE CONTUSIONS AND MUSCULAR STRAIN TO HIS LEFT RIBS, SIDE AND BACK.
