Showing all 5IE was climbing down the ladder on the jaw feeder when his foot slipped on the ladder and his leg went thru the ladder and he fell backward. The IE suffered a groin muscle pull.
The employee placed a ladder against a conveyor with the top into the framework. When he was climbing the ladder to tie it off it slipped out from underneath him. He was 3/4 of the way up the ladder when it slipped & he fell approx. 8' to the concrete pad suffering a compound fracture to his arm, contusions to his ribs & a fracture in his foot.
MISSED LAST STEP COMING DOWN OFF LOADER AND FELL.
EMPLOYEE WAS WELDING A GATE FOR A SPLITTER BOX. THE LADDER WAS NOT SET UP PROPERLY AND HE FELL.
