Fall from ladders

The employee placed a ladder against a conveyor with the top into the framework. When he was climbing the ladder to tie it off it slipped out from underneath him. He was 3/4 of the way up the ladder when it slipped & he fell approx. 8' to the concrete pad suffering a compound fracture to his arm, contusions to his ribs & a fracture in his foot.

JAW CRUSHER WAS NOT TAKING FULL FEED. EE EXITED LOADER TO CHECK CRUSHER CHAMBER AND AFTER NOTICING A TREE ROOT INSIDE, CLIMBED DOWN, LOCKED IT OUT AND CLIMBED BACK UP ON PLATFORM TO FREE ROOT. HE HAD DIFFICULTY, SO CLIMBED IN JAW AND SLID DOWN, CATCHING BOOT AND LODGING LEF IN JAW. TWISTED LEFT KNEE AND HEARD IT POP.
