While crawling out from a screen deck, employee dropped one foot to scaffold board. The board broke and employee fell, injuring right middle finger, and slid down chute to the conveyor belt.
Employee lost balance and fell from back loader platform.
Employee slipped while dis-mounting dozer. The employee caught himself with one arm; thus straining his back. Refused medical treatment at first. Employee then went to the doctor for observation (No medication, No restrictions, No therapy). The employee then went to the doctor again (12/4/06) and received physical therapy treatment to resolve issue.
Employee stepped off steps, slipped on gravel, hurt (L) knee.
