A maintenance repair person was tasked with replacing a transfer pump on the grinding aid tank. The employee stepped back, and EE's right leg entered an opening in the grating. The employee was also experiencing pain in the groin area when lifting. After seeking medical treatment, the employee was diagnosed with a hernia. The employee had surgery on November 25, 2019.

Showing all 10On June 15, 2019, Employee stepped on a golf ball sized rock on the barge. The employee was attempting to secure a barge when they stepped on rock causing them to roll their ankle. On July 2 2019, a physician prescribed PT and a walking boot.
Mobile equipment mechanic was attempting to fill hydraulic oil in a bull dozer. The EE was standing on the bed of the service truck when EE turned to the left. The employee accidentally stepped off of the bed fell to the ground. The employee was diagnosed with a wrist strain/sprain and an elbow contusion. EE was given a brace for the wrist and was placed on restricted duty.
EE was walking on an access platform of a mill ball sorter. The ee missed a step and caught self with left arm to avoid falling. The ee felt a pain in left shoulder. Employee sought medical attention the day of injury and was released to full duty. After a follow up evaluation with a orthopedic on 5/8/2017, the employee was placed on restricted duty.
