Employee was walking around the ANFO loader when he tripped over a rock and landed on his left elbow. EE had a metal plate surgically implanted in his left elbow a few years prior in an injury unrelated to the mine or current case. The 9/9/08 incident aggravated the ee's left elbow & surgery to remove the plate implant was performed on 11/7/08.

He did not request medical attention at the time of the incident. Subsequently, he was examined at his request by a medical doctor on 8/25/2008 and was placed on restricted duty.
Employee was cleaning a guarded catwalk deck and fell into a hole made for debris disposal. Employee sustained contusions, sprain, and laceration.

EMPLOYEE CLIMBING INTO POSITION TO ADD HYDRAULIC FLUID TO SCALER STEPPED DOWN & SLIPPED IN SOME OIL & FELL. HE SAID HE TWISTED HIS LEFT FOOT, STRUCK LEFT ELBOW & CUT HIS LEFT THUMB. AT TIME OF ACCIDENT HE DID NOT INDICATE THAT ANY MEDICAL ATTENTION WAS NECESSARY.
SLIPPED OFF OF TRACK ON BUSTER WHILE FUELING ANDHURT LOWER BACK. THERE WAS GREASE AND OIL ON HISBOOTS.
IN THE MINE AT MAIN S13 REPLACING HYRAULIC HOSE, STEPPED ON GUARD THAT HAS BEEN REMOVED. SLIPPED & FELL, SEVERE CONTUSIONS/SPRAINED LEFT ANKLE WEAR AIR CAST
EE WAS SITTING ON A 12" BLOCK WELDING ON PARTS IN FRONT OF HIM. HE WENT TO REPOSITION HIMSELF AND LOST HIS BALANCE FALLING ONTO HIS LEFT SIDE. THE LEFT SIDE OF HIS MID-SECTION STRUCK A ROCK AS HE LANDED.
REPLACING WEAR PLATE ON HP300 FEEDER. WAS WALKING AROUND THE FEEDER TO BURN OFF BOLT, STEPPED OVER BEAM ONTO A ROCK AND TWISTED ANKLE. TREATMENT INCLUDED THE USE OF SPLINT.
