Minor was moving a 55 gallon drum utilizing a hand dolly designed to do so. Minor slipped and fell onto left side. Minor complained of left shoulder pain and later taken for medical care. This has been first aid with employee being observed and not restricted of any duty. Ultimately on November 4, 2019 employee underwent surgery on left shoulder resulting in loss time from work.

Showing all 20Employee was cutting a bolt. The employee lost his footing, grabbing for the guard, the jagged edge of the guard cut the left forearm.
EE was climbing down from the operator cab of a loader when he lost his balance and fell to the ground. Was taken to ER on 11/6/11 for x-rays and they were negative. Continued to have pain and a MRI was ordered on 11/15/11. EE was diagnosed on 11/18/11 with a talus fracture.
Weather was cold and wet. Employee was exiting loader at the end of his shift, when his foot slipped on the slick step. This allowed his weight to shift, causing a sprain to the lumbar region. Employee did not fall.
Ee stated that he slipped and fell inside the wash plant while attempting to change screens. Ee did not report injury until 5 days later. Ee has a bruised rib.
Employee slipped and fell while tring to climb up the latter of the loader he was operating. Employee sprained his left wrist from the fall, apperently due to oil on his boots from the fuel area.
EE WAS REMOVING A 1" PIECE OF METAL PLATE FROM INSIDE OF THE SCRAP DUMPSTER. HE HAS POSITIONED IT ON TO THE FORKLIFT, WHEN HIS LEFT FOOT SLIPPED, CAUSING HIM TO COME IN CONTACT WITH THE EDGE O F THE METAL PLATE. IT CAUSED A LACERATION ON HISLEF CALF AREA THAT REQUIRED STITCHES.
EE WAS WALKING DOWN THE STAIRS ON THE WEST SIDE TOP OF THE PRODUCTION SCREENS, HE THEN SORT OF SLIPPED TWISTING HIS FOOT, HE DIDN'T THINK HE STEPPED ON A ROCK THOUGHT IT WAS THE WAY HE STEPPED.
I WAS CLIMBING DOWN INTO THE POSSUMBELLY OF THE TYROCK SCREEN. I SLIPPED AND SLID DOWN ABOUT 3'. WHEN MY RIGHT FOOT HIT THE CONVEYOR BELT, I DISLOCATED MY RIGHT ANKLE.
EE WAS WORKING ON CHUTE, WHILE CLIMBING DOWN, HECAUGHT HIS SLEEVE OF HIS OVERALLS ON THE CHUTE, CAUSING HIM TO SPIN AROUND HITTING HIS RIBS.
EMPLOYEE WAS WALKING ON THE GROUND FROM CRUSHERS3 AND 4 TOWARDS TOWER #2, WHEN HE TWISTED HIS ANKLE. THE AREA AROUND WHERE HE WAS WALKING WAS CLEAR OF DEBRIS.
EMPLOYEE WAS WASHING EQUIPMENT IN THE WASH BAY. HE WAS STANDING ON A PLATFORM LADDER HOLDING A WAND FOR WASHING. HE STEPPED BACK THINKING THAT THE METAL WAS THERE AND THERE WAS NONE. IT WAS FO GGY AND HARD TO SEE WITH GREASE AND DIRT COMING OFF THE MACHINE. HE FELL TO THE FLOOR AND LANDEDON SIDE. HARD TO BREATHE, CUT HIS EAR.
REPAIRING ON PLANT, WALKED BY SHAKER, BUMPED RIGHT KNEE.
EE WAS CLIMBING UP LADDER ON 992C LOADER. LADDER HAD MORNING DEW ON IT CAUSING HIS FOOT TO SLIP AND FALL TO GROUND FROM APPROX. 10' (SECOND RUNG FROM THE TOP).
EMPLOYEE STEPPED OFF STAIRS ONTO ROCK, STUMBLED AND FELLS, STRIKING HIS LEFT KNEE ON A ROCK.
EMPLOYEE DOING MAINTENANCE WELDING UNDER SHAKER/SCREEN ON SHOOT. WELDED OR TACKED PIECE OF METAL ON SHOOT. STOOD ON TACKED PIECE OF METAL, TACK OR WELD BROKE CAUSING EMPLOYEE TO SLIP AND FALL 2 TO 2 1/2 FEET. EMPLOYEE DID NOT GO TO DOCTOR UNTIL 11/6/01 AT HIS REQUEST.
EE WAS WALKING AND TRIPPED OVER A ROCK TWISTING HIS RIGHT KNEE.
EE WAS WALKING UP STAIRS WHEN HE TRIPPED ON STEPAND FELL HITTING HIS RIGHT SHOULDER.
EMPLOYEE SLIPPED AND FELL IN A PUDDLE OF OIL IN THE FUEL BAY.
EMPLOYEE WAS SERVICING PLANT, SLIPPED AND GRABBED HOLD OF STRUCTURE TO KEEP FROM FALLING. EMPLOYEE DID NOT SEE A DOCTOR UNTIL 5-16-01.
