Operator said EE slipped & fell backwards against loader bucket. Said EE was ok but would go to clinic to be sure. info previously reported to MSHA on 7/8/16. Operator returned to work the following AM and indicated no restrictions, no med's, etc and went to work. Subsequent misrepresentations - info provided to MSHA inspector

Went to the Clinic - Diagnosis says a Left radius fracture - Soft cast applied and referred to orthopedic for review. Able to work with limitations
Tripped and fell going out of office door and down 5" step to ground level. Right leg and back pain.
