Employee misplaced EE's foot while descending a ladder and slipped of the rung. EE dropped 3-4 feet and landed on EE's right foot. EE's ankle folded over and sprained it.

Operator said EE slipped & fell backwards against loader bucket. Said EE was ok but would go to clinic to be sure. info previously reported to MSHA on 7/8/16. Operator returned to work the following AM and indicated no restrictions, no med's, etc and went to work. Subsequent misrepresentations - info provided to MSHA inspector

Operator was done for the day. Parked loader and shut it down. While climbing down from loader, he accidentally missed a step. He tried to catch himself and fell on his hand. Went to the Clinic - Diagnosis says a Left radius fracture - Soft cast applied and referred to orthopedic for review. Able to work with limitations
