Employee misplaced EE's foot while descending a ladder and slipped of the rung. EE dropped 3-4 feet and landed on EE's right foot. EE's ankle folded over and sprained it.

Operator said EE slipped & fell backwards against loader bucket. Said EE was ok but would go to clinic to be sure. info previously reported to MSHA on 7/8/16. Operator returned to work the following AM and indicated no restrictions, no med's, etc and went to work. Subsequent misrepresentations - info provided to MSHA inspector

Operator was done for the day. Parked loader and shut it down. While climbing down from loader, he accidentally missed a step. He tried to catch himself and fell on his hand. Went to the Clinic - Diagnosis says a Left radius fracture - Soft cast applied and referred to orthopedic for review. Able to work with limitations

Tripped and fell going out of office door and down 5"" step to ground level. Right leg and back pain.

WAS LAYING ON FENDER AND FRAME OF FINDLAY PLANT & REACHED DOWN TO PULL WRENCH AND EE FELT A POP ON RIB CAGE, LEFT SIDE.

LIFTING FROST WITH LOADER AND THE WEIGHT JOLTED HIS BACK.

WAS DRIVING FRONT END LOADER AND IT WAS A REALLYRUFF AREA TO BE WORKING IN. BOUNCING UP AND DOWNCAUSED SEVERE BACK AND RT. LEG PAIN.

WIND BLEW METAL GATE INTO RIGHT LEG SCRAPING SHINBONE ON 2-24-02. LEG BECAME INFECTED BY BLUE JEANS RUBBING ON LEG (ALLERGIC TO DYE IN JEANS). WENT TO HOSPITAL ON 3-10-02 FOR TREATMENT OF INFE CTION OF RIGHT LEG.

EE WAS WORKING ON FIELD HOPPER TAKING BIG ROCKS OFF OF GRATE ONTO THE GROUND. HE STRAINED GROIN AREA.
