Employee stated that he looked to see what caused the accident and he had stepped in a hole and it was not visible due to water. Doctor took him off work since he had a fracture.

Employee was walking when he stepped in a hole and his foot slipped back causing pain in his left calf muscle.
Employee was on the roofbolter, after he handed a new bolt and plate to an employee to install, he went to step down off of roofbolter twisting his left ankle causing sprain to ankle.
