At approximately 2:30 a.m. on 06/04/2015, employee drove the vehicle to the upper plant gate and as he was locking the gate, he noticed that the vehicle was rolling and as he ran to catch it, he fell injuring his neck and lacerating his forehead.
The serviceman had finished servicing the D10N Dozer, stepped off of the push arm, and while holding onto the handhold, he stepped onto the ground while turning around and twisted his back resulting in a muscle sprain.

The employee was watching another employee hydroseeding with the hose when the other employee lost his footing & stumbled & employee ran to assist the other employee, fell and rolled down the slope, pulling the muscles in his neck.

WHILE CLIMBING OFF D9N DOZER EE FELL FROM ARM ON BLADE TWISTING LEFT KNEE.
