Employee was in the process of dragging 48" top structure for the belt move they were making. He stepped on another piece of structure causing his knee to turn causing him to fall.

THE ORIGINAL ACCIDENT OCCURED MARCH 6, 2004 AT WHICH TIME EE CONTINUED TO WORK. HE WAS EXITING THE SURFACE MANTRIP, SLIPPED & FELL TO THE GROUND, HURTING HIS BACK. HE WORKED THROUGH JUNE 18, 2004 & THEN BEGAN MISSING WORK DUE TO BACK PROBLEMS.
EMPLOYEE WAS IN THE SCOOP. WHEN HE MADE AN ATTEMPT TO GET OUT OF THE SCOOP HE SLIPPED AND FELL ONTO THE SCOOP TWISTING HIS BACK (HIS ARM SLIPPED WHILE HOLDING TO GET OUT).
EE WAS ALLEGEDLY TREATING COAL BED FOR COLD WEATHER, SLIPPED AND FELL. NO EVIDENCE OF INJURY AND EE WAS NOT DIRTY. THERE WAS NO WITNESSES. I STRONGLY FEEL THIS ACCIDENT SHOULD BE DISREGARDED.
EE WAS HELPING THE #14M TO SET UP. AFTER GETTING THE MINER TO LOCATION, EE WAS LAYING MINER CABLE OUT OF THE ROADWAY. WHEN HE BENT OVER TO LAY THE CABLE TO THE SIDE, HE LOST FOOTING, FELL ON H IS LEFT HAND AND FRACTURED HIS RING FINGER.
WHILE TRAVELING TO THE CM, EE STATED HE STEPPED ON THE MINER CABLE AND HIS KNEE WENT OUT. THIS OCCURRED IN APRIL OF 2001 TO THE RIGHT KNEE RESULTING IN CORRECTIVE SURGERY. EE SUFFERS FROM DEG ENERATIVE JOINT PROBLEMS.
EE WAS CLIMBING OUT OF SCOOP AND HIT HIS ELBOW ON THE DIRECTIONAL BOX IN THE DECK. TONEY IS 9-5-02 UPDATED INFORMATION WAS RECIEVED TODAY THAT HE WAS GOING TO BE A LOST TIME DUE TO MEDICAL PRO BLEMS WITH THIS ELBOW.
EE WAS MOVING CABLE OUT OF THE WAY, WHEN A ROOF BOLT FELL, TRIPPING CABLE SWITCH (CABLE REEL) CAUSING EE TO FALL TO THE GROUND AND HITTING ON HIS LEFT ELBOW.
