EE WAS WASHING THE DRILL TO PERFORM MAINTENANCE WORK ON IT. HE STOOD ON THE DRILL DECK PLATE (APPROX 4' X 4') WHICH HAD MOVED WHEN MAST WAS LAID DOWN. THE PLATE TIPPED WHEN HE STEPPED BAKC AND HE FELL THROUGH IT. HE TRIED TO BREAK THE FALL WITH HIS RT HAND AND HURT HIS SHOULDER.

EMPLOYEE WAS ATTEMPTING TO CLIMB UP ON ROTARY DRILL HEAD ON DRILL RIG TO CHECK ON OIL LEAK. HE REACHED FOR A GRABHOLD, MISSED, SLIPPED ON SLICK SURFACE AND FELL APPROX. 7 FEET TO MAIN DECK ON DRILL.
