Showing all 48Was tramming #10 mantrip inby traveling and heard a loud noise that came from the batteries. Stopped the mantrip, knocked the breaker, set the brake and got out of the mantrip. Stood up his foot slipped causing left knee to twist and pop. Has starting PT on 8/12/10 therefore making it a reportable accident. Member continues to work as scheduled.
Member was just underground approximately 300 feet when explosion occurred deep into the mine, when he stated he ran and twisted his right knee and inhalation of gas.
Attempting to start pump located behind splitter box. When he attempted to walk around the splitter box through approximately 8 inches of water, his left knee gave out. Marfork Safety Department was informed employee returned to work but he called today 2/1/10 and reported he is scheduled for surgery 2/17/10.
Member was walking backwards caarrying a top roller 48" belt structure when he tripped and fell landing on his backside. The top roller struck his lower left leg causing a sprain/contusion.
We were marking off belt line for the longwall belt. We had to use a 12' ladder due to the height of the mine roof, while moving the ladder under some cables to reposition it the ladder slipped. While I was still holding onto ladder. Tearing rotary cup in my right shoulder.
While the member was carrying roof bolts from the middle of the machine toward the front tray of the bolter, he stepped in mud and tripped. The roof bolts he was carrying hit the rib keeping him from falling causing a strain to the middle portion of his back.

Had X-rays which showed no evidence of a fracture and was referred to a specialist who diagnosed him with a severe sprain and removed him from work.

HE TRANSPORTED HIMSELF OUTSIDE AND LEFT VIA AMBULANCE TO CHARLESTON GENERAL HOSPITAL WHERE X-RAYS WERE PERFORMED AND WERE NEGATIVE. HE WAS DIAGNOSED WITH TORN TENDONS BY HIS SPECIALIST.
Employee exited a Brookville motor in an attempt to place a mine phone along the track entry. He stepped up on a flat car and as he stepped off his right ankle rolled over producing a loud popping noise. The safety department was not notified of this accident until 12/2/05.
Ee was carrying a acetylene bottle from last open break toward tool cars. He tripped over half header and fell face first toward the mine floor. The actylene bottle fell against ee rt. hand.His rt middle finger was cut off at 1st joint and his rt. ring finger was cut and mashed. Finger was put in ice and sent to hospital. Unable to reattach finger.
EE WAS WALKING AROUND HEAD DRIVE & STEPPED ON #1 SHIELD RELAY BAR, SLIPPED & FELL. HE DID NOT SEEK MEDICAL ATTENTION UNTIL 8/17/05. DUE TO CONTINUAL EXPOSURE THIS INJURY HAS BEEN AGGRAVATED & HE IS NOW SEEKING MEDICAL ATTENTION. THIS INJURY IS BEING REQUESTED TO BE FILED AS AN OCCUPATIONAL ILLNESS.
INJURED WAS GETTING OUT OF #11 JEEP. HE PUT HIS LEFT LEG OUT FIRST WHEN IT BECAME STUCK IN THE MUD. THE AREA WAS WET AND MUDDY. AS HE TRIED TO GET HIS FOOT OUT OF THE MUD BY TWISTING AND PULLING UP, HE HEARD SOMETHING POP IN HIS LEFT KNEE.
INJURED WAS DISMOUNTING FROM NW#2 BELT. HE HAD PLACED HIS LT. FOOT ON A TOP CONVEYOR RAIL WHILE HOLDING ONTO A STRUCTURE SUPPORT CHAIN. AS HE BEGAN TO STEP DOWN WITH HIS RT. FOOT, HIS LT. FOOT SLIPPED OFF THE RAIL & IN BETWEEN BELT & STRUCTURE, CAUSING HIM TO FALL. THE AREA WAS WET.
INJURED STOPPED TO USE A TELEPHONE. WHEN HE HAD FINISHED HE WAS CROSSING BETWEEN TWO PARKED LOCOMOTIVES. HE STEPPED UPON THE COUPLERS WHEN HIS FOOT SLIPPED OFF CAUSING HIM TO FALL AGAINST THE FRONT BUMPER.
Injured was stepping out of a cont miner conveyer pan. As he put his right leg out of pan onto mine floor he felt a popping in his right knee. He was later diagnosed with soft tissue damage in the knee.
INJURED WAS WALKING BESIDE #1 HEAD DRIVE MOTOR ON LONGWALL 17 WHEN HE STEPPED IN A HOLE, CAUSING HIS RIGHT KNEE TO BE TWISTED. HE CONTINUED TO WORK, BUT HIS KNEE BEGAN TO SWELL. ON 3/24/04 RESULTS FROM MRI REVEALED SEVERE SPRAIN. HE IS SCHEDULED FOR THERAPY BEGINNING ON 3/26/04.
INJURED WAS EXITING A 988 END LOADER WHEN HIS RIGHT FOOT SLIPPED OFF THE NEXT TO LAST STEP. THIS CAUSED HIM TO LAND AWKWARDLY ON HIS RIGHT LEG. HE CONTINUED TO WORK BUT HIS KNEE DIDN'T GET ANY BETTER. HE STARTED TO MISS 12/15/03 AFTER SCHEDULED DAYS OFF. HE THOUGHT HIS KNEE WOULD GET BETTER.
EE WAS EXITING A MANTRIP ON THE SURFACE, HIS TOOL POUCH CAUGHT MANTRIP WEBBING CAUSING HIM TO FALL BACKWARDS. HE ATTEMPTED TO GET UP BY PUSHING OFF WITH HIS LEFT KNEE WHEN HE HEARD SOMETHING POP.
AFTER PLASTING A STOPPING EE WAS TAKING OFF HIS RUBBER GLOVES WHEN HE LOST HIS BALANCE & FELL AGAINST A COAL RIB. HE GRABBED HOLD OF A SUPPLEMENTAL RIB SUPPORT (PIZZA PAN) THAT CUT HIS HAND
EE WAS WALKING DOWN AN INCLINED ROADWAY WHEN HE STEPPED IN A HOLE THAT CAUSED HIS RT. KNEE TO BE BENT BACKWARDS. HE CONTINUED TO WORK BUT HIS KNEE DID NOT IMPROVE. ON 3/7/03 HE HAD HIS KNEE OP ERATED ON.
EE WAS LEAVING THE FACE AREA OF THE LONGWALL 16 SET-UP AREA. AT #1 GATE SHIELD HE TRIPPED OVER THE RELAY BAR AND HIT HIS LEFT KNEE.
EE WAS ATTEMPTING TO STEP ON THE ROCKBOX NEAR THE #2 TRIPPER ON LONGWALL 15 BELT WHICH HAD BEEN TAKEN OUT OF SURFACE TO PREPARE THE AREA FOR CUTTING. HIS RIGHT FOOT SLIPPED, CAUSING HIM TO FALL AND TWIST HIS ANKLE.
EE WAS HANDLING ROCK DUST HOSE ALONG THE BELT STRUCTURE ON N MAINS WHEN HE STEPPED IN A HOLE AND TWISTED HIS KNEE. HE WAS NOT SCHEDULED TO WORK, AND BELIEVED IT WOULD BE ALL RIGHT. HE CALLED H IS SUPERVISOR 6-10 AND STATED HE WAS GOING TO THE DOCTOR. HE WAS DIAGNOSED AS HOAVE A DISLOCATED PATELLA.
EE WAS CARRYING A WELDER TO THE BACK OF THE WAREHOUSE. EE TRIPPED, AND FELT PAIN IN HIS LOWER BACK.
EE HAD BEEN OPERATING AN IT28 END LOADER TO LOADSUPPLIES HE HAD DEENERGIZED THE MACHINE & WAS DISMOUNTING AS HIS RIGHT LEG TOUCHED THE GROUND HE TWISTED HIS RIGHT KNEE. HE DR INDICATED TO HI M THAT THIS WAS AN AGGRAVATION OF A PREVIOUS IN JURY. ADDITONAL MEDICAL INFORMATION WILL FOLLOW
EE WAS WALKING OUT OF THE #1 ENTRY ON HEADGATE 16 SECTION. AS HE TURNED THE LEFT CORNER INTO THE LAST OPEN CROSSCUT, HE SLIPPED AND FELL TO THE MINE FLOOR, TWISTING HIS KNEE AS HE FELL. EE IS AWAITING APPROVAL FOR DIAGNOSTIC TESTING ON HIS KNEE.
EE HAD PUT THE SCOOP ON CHARGE FOR THE NEXT SHIFT. HE STEPPED OUT OF THE SCOOP AND LOST HIS FOOTING. HE FELL, STRIKING HIS HEAD AGAINST A NEARBY FORKLIFT HEADLIGHT. HIS NOSE WAS BROKEN BY THE IMPACT.
EE HELPING PUT CRIB BLOCKS UNDER A FEEDER TO REPLACE CRAWLER LIFT JACK PIN. HE STOOD UP TO GO AROUND TO OTHER SIDE, TRIPPED OVER A CRIB. HE FELL STRIKING HIS RIGHT KNEE AGAINST A ROCK 1" X 10" . HE CONTINUED TO WORK UNTIL 3-2-02 WHEN HE WENTTO CAMC FOR HIGH BLOOD PRESSURE. DR. NOTICED KNEE & SUGGESTED SPECIALIST. HAS NOT SEEN SPECIALTIST, NO MEDICAL TREATMENT OR LOST TIME AS OF YET
EE WAS WALKING DOWN THE LONGWALL TRACK ENTRY WHEN HE STEPPED INTO A HOLE, HIS BOOT GOT CAUGHT, PRODUCINT A TWISTING ACTION ON HIS RIGHT KNEE.
THE LONGWALL SECTION WAS MOVING TO A NEW SET UP.EE AND OTHERS WERE BUILDING CRIBBS AS SHIELDS WERE BEING REMOVED. AFTER #30 SHIELD WAS REMOVED, A ROOF BOLT BROKE AND THEY RAPIDLY GOT UNDER OTHER SHIELDS FOR COVER. IN PROCESS EE TRIPPED AND FELL CAUSING HIM TO FALL ON HIS CAP LIGHT BATTERY.
EE WAS WALKING BY ROCK DUSTER & TRIPPED & TWISTED KNEE WHEN HE FELL.
INJURED WAS GETTING OF LOADER WHEN HIS FEET HIT THE GROUND HE STEPPED ON A STITCH CAUSING HIM TO FALL AND HURTING HIS WRIST AND ANKLE.
EMPLOYEE WAS MOVING 1/2 HEADUS IN BACK OF 1 TON SUPPLY TRUCK, MAKING ROOM TO LOAD OMEGA BLOCKS. HE BENT DOWN AND PUT ONE HAND ON RT SIDE RAIL OF TRUCK, IT FOLDED DOWN CAUSING HIM TO FALL TO GROUND.
EE HAD JUST FINISHED DRILLING A BOLT HOLE & WAS WALKING BACK TO THE CONTROLS TO MOVE THE BOLTER UP FOR THE NEXT ROW WHEN HE TWISTED HIS RIGHT KNEE. EXAM BY ORTHPEDIC SURGEON INDICATES DEGENE RATIVE ARTHRITIS
EE WAS WALKING TOWARD MANTRIP, WHEN HIS FOOTING SLIPPED CAUSING HIM TO FALL DOWN, TWISTING HIS RIGHT LEG. MEDICAL DIAGNOSIS REVEALED HIS RIGHT FIBULA HAD BEEN FRACTURED.
EE HAD HAD PREVIOUS TROUBLE WITH HIS KNEE AS HE WAS WALKING UP STEPS AT THE MINE OFFICE, HIS LEFT KNEE GAVE OUT CAUSING HIM TO ALMOST FALL.
EE WAS SETTING TIMBERS WHEN HE TURNED AROUND AFTER GETTING A TIMBERS HE TRIPPED OVER A BUNDLE OFWEDGES CAUSING HIM TO FALL TO MINE FLOOR TWISINGHIS BACK IN AN AWKWARD POSITION
EMPLOYEE WAS WORKING ON HG14 ENTRY #2 AT SPAD 12318. HE WAS ON THE SECOND STEP OF A 4' LADDER, CLIMBING UP TO SET THE SPAD. HE CHECKED THE LADDER BEFORE CLIMBING UP. HOWEVER, THE LADDER BECAME UNSTEADY AS HE BEGAN TO CLIMB CAUSING HIM TO LOSE HIS BALANCE, HE FELL LANDING ON HIS RIGHT LEG. INJURIES WERE A FRACTURED TIBIA AND CRACKED FIBULA.
