Employee was in the process of examining the beltline and twisted right knee.
Employee exited mantrip at the end of shift and felt pain in lower back and left leg.
Employee was assisting the unit mechanic with changing shuttle car tire, the tire slipped causing the employee to fall on the tire, striking the wheel rim just above EE's right eye, resulting in 5 sutures.
While stepping out of operator's deck of battery scoop, employee tripped on the seatback and caught them self with their right hand resulting in a slight fracure.
Employee had moved shuttle car, while stepping out of the car, he slipped, catching himself with his left hand, resulting in a small laceration and the fracture of his left ring finger.
Miner was pulling water hose thru a permanent stopping when he tripped over a concrete block on the mine floor. Miner fell to mine floor striking right wrist on additional concrete block, causing a wrist fracture.
While positioning a step ladder to inspect a holding tank, the ladder slid forward and started to fall, the employee attempted to secure himself by grabbing the ladder rail, this entangled his wrists in the ladder railing. The ladder and employee fell to the ground.
Employee set up a 8 step ladder on unstable ground. The side he was climbing was on concrete at the other side was in loose mucky coal. Employee went to 3rd rung to check footing of ladder and ladder begin to sink. Employee jumped off ladder and broke his right ankle.
Employee was on scoop unplugging the batteries when he lost his balance and fell off the scoop. He caught his right knee in the fender well resulting in a meniscus tear.
Employee was in the process of moving an oxygen and acetylene cart when he stumbled and fell. The cart fell across the side of his right knee and leg, fracturing his lower leg.
Walking out of shower, slipped and fell chipping tooth.
Loading bolter, fell over pins laying in crosscut. 5' bolts were on the ground. As employee approached the bolts, he tripped on a bundle his right leg landing on another bundle. This caused a 1" x 4" laceration that required 43 sutures.
While attempting to re-hang a pinner cable, the roof bolter moved up in the next place over, not knowing the cable reel was locked. Cable came down, knocking employee to the ground. Incident only, finished shift. Wasn't reported to Safety Dept until 05/15/2012 Hernia surgery on 6/19/2012 making this a lost time accident.
Employee was on the engine compartment of #8 Getmam Tractor removing the cover bolts, Employee slipped off the tractor and landed on his left knee. Worked until knee surgery on 09/14/2012
Opening 6'x6' door in stopping stepped backward and left knee popped. Worked full duty until knee surgery on 6/14/2012 making this a lost time accident.
Getting out of the operators compartment of #2 diesel scoop lost his footing and fell. While trying to catch himself he hit his arm on the deck of the scoop. Fracturing both bones in his left forearm.
Walking on belt line said too much weight on right leg, slipped and fell on back. Had pain in back and right leg was trying to step up on walk board which was wet and muddy.
Employee was stepping over a ladder laying on the mine floor when he felt a pain in his right hip and lower back.
Employee was sitting on a man trip when he was pulled out of the man trip by a mine employee fracturing his right ankle.
Traveling on foot through mandoor, fell when foot hung on the door framing, causing left side of body to strike concrete block lying on ground.
Employee tripped over pipe, sprained right foot.
While trying to stand from a kneeling positon the employee stepped on his shoestring, lost his balance and fell back to the ground hitting his knee on a pin plate attached to a roof bolt he was fixing to install. The employee received 15 sutures.
While pulling up miner cable the employee's foot got tangled up in the miner cable and when the cable pulled tight it caused the employee to fall forward striking his left knee against the miner.
While cleaning mud out of the shaker screen the employee twisted his left knee. The employee had surgery on 3-22-10. This accident did not become Lost Time until 3-22-10.
While standing on a continuous mining machine and trying to hang a piece of ventilation tubing at the inner seam slopes, the employee stepped backward off of the miner and landed on his self rescuer bruising his lower back.
The employee was carrying timber when he tripped over a wiper box, twisting his back.
Employee was working on top of five foot scaffling in the #2 inner seam slope. As he was getting down off the scaffling he fell on his back onto an I-beam that was laying on the mine floor causing an injury to his lower back. (Misc Mail 6/29/09 #3-this did not become a lost time accident until 6/22/09)
While walking to the ride, the employee slipped in the mud and fell down, injuring her lower back. This injury did not become lost time until 5-19-2009.
While working on a shuttle car, employee stumbled on loose rock and fell on left wrist. Employee fractured left wrist. Surgery required.
Employee was getting out of the shower located in the bathhouse when she slipped and fell causing a laceration to her left leg. She received 2 sutures.
Employee was walking on the #3 unit and stumbled causing him to fall to the ground injuring his right foot.

Employee was getting off a Getman Tractor when he slipped causing him to fall on his left side causing a strain/sprain to his left shoulder. This did not become a Lost Time Accident until 2/13/2009.
Employee was stepping out of the operator's compartment of a diesel scoop when his right foot slipped causing pain in his left leg. Upon completion of medical tests it was determined his lower back was initially injured and was the cause of the pain in his left leg. This did not become a Lost Time Accident until 6/6/2008.
Employee was pre-shifting on #4 unit in the face of the #3 entry when he stepped in a hole, in the mine bottom, causing him to strain his left knee.
Employee was walking toward the bottom of the Draw off tunnel when he slipped on a small piece of ice causing him to hyperextend his left knee causing a strain/sprain to the knee. This did not become a Lost Time Accident until 5/5/08.
Employee was walking from the office trailer to the warehouse when he slipped and fell on the pavement injuring his left shoulder. This did not become a Lost Time Accident until 4/16/2008.
Employee stated: that he picked up a box of pinner glue, turned around falling to the ground twisting the right knee. This injury did not become a lost time until 9/24/07.
Employee was roof bolting in the face of #7 entry on #1 unit when his left knee slid on a small piece of rock, that was on the ground, causing his knee to twist. This did not become a Lost Time Accident until July 3, 2006.
While knocking the breakers on the power boxes, employee slipped and fell. He reached out with his right hand to catch himself and his middle finger folded back, causing a fracture.
While walking across the parking lot the employee stepped on something and twisted his ankle causing him to fall and hit his right knee on the asphalt. Did not become a lost time until 3/17/06.
While walking between #9 and #10 crosscuts employee twisted her leg behind her knee. This accident did not become lost time until 04/12/2006.
Employee was crossing an overcast. As he stepped down on the other side his left knee popped and he felt a sharp pain in the knee. This did not become a Lost Time Accident until 2/28/2006.
Employee was getting out of a Shuttle Car and twisted his right knee. This did not become a Lost Time Accident until 11/30/2005.
Employee had his left leg on top of a battery scoop while inserting the plug of a battery powered rockduster into the scoops PTO. His leg slipped striking it against the battery scoop causing a contusion to the shin.
Employee is a roof bolter operator. He had drilled a hole in the mine top to install a roof bolt. After he inserted the roof bolt he was attempting to straighten the bolt out to finish the installation process by pushing on the roof bolt. This caused him to loose his balance causing his left knee to twist. (This did not become reportable until 07/26/04.
WALIKING ON RETURN SIDE OF A3 BELT HEADER AND SLIPPED ON A MUDDY SURFACE CAUSING HIM TO FALL ON HIS LEFT SIDE.
EMPLOYEE WAS TIGHTENING ROPES ON #1 UNIT TAILPIECE AND SLIPPED ON WET GOB TWISTING HIS RT. KNEE. (THIS DID NOT BECOME REPORTABLE UNTIL 4/12/04).
GETTING OUT OF SCOOP DECK & CAUGHT HIS RIGHT FOOT UNDER A PANEL BOX IN DECK CAUSING HIM TO TWIST HIS RIGHT KNEE AS HE FELL OUT OF THE DECK.
