Showing all 24EE stepped off back of roof bolter to hang bolter cable. While holding onto the bolter with his hand, it slipped and he fell against a pile of strap, cutting his arm.
While carrying belt structure to track entry, EE got his foot stuck in mud and twisted his back.
Prying on jack pin, slate bar slipped twisting his right shoulder. He fell to the ground on same shoulder. Began missing work on 10/12/08. He had surgery.
Stumbled while walking around scoop and twisted ankle. (Adverse conditions - busted bottom and mud made walking difficult).
Employee was getting off the bus, fell and landed on his back on the SCRS.
Injured was walking down steps from mine office trailer. Left foot slipped on wet mud on step and left foot slid back under the step. When foot slipped, weight trasnferred forward while foot was wedged between steps. Caused bone in left leg to fracture.

while servicing roof bolter, was going around roof bolter and stepped on pc. of curtain a lump of coal was underneath it causing employee to twist left knee. Accident happened 11-16-2006 at this time the employee did not miss work. He had surgery on his knee on 02-20-2007 and started missing work at that time.
While getting out of coal hauler to help move miner, stepped wrong and twisted left ankle
Spraying miner off, waiting on coal hauler. Heard CH coming, got off miner; left foot got hung on cable to junction box slipped and fell off miner.

STEPPING OFF A LADDER AND STEPPED IN A HOLE AND HURT HIS ANKLE.
EE WAS GETTING OFF MANBUS; TRIPPED AND FELL ON HIS FACE INJURING HIS NOSE. EE SUSTAINED A NON-REPORTABLE (NOSE) INJURY ON OCTOBER 22, 2003, WHILE AT WORK. HE HAS HAD MINO R OUT-PATIENT SURGERY ON 12-04-03 AT WHICH TIME WE ARE CHANGING THE STATUS OF HIS INJURY TO REPORTABLE.
EMPLOYEE WAS TIGHTENING BELT ROPES ON #2 BELT WHEN THE CHAIN THAT THE RATCHET WAS HOOKED ON BROKE, CAUSING HIM TO FALL, HITTING HIS HEAD ON THE SCOOP BUCKET, CUTTING OVER HIS LEFT EYE. EE WENT TO THE HOSPITAL RECEIVED 4 STITCHES.
EE STEPPED OFF COAL HAULER, KNEE POPPED.
EE WAS CARRYING BELT ROLLER TO REPLACE ROLLER ON BELT. HE TRIPPED & FELL ON ROLLER, SPRAINED RT KNEE. 3/4/02 1ST DAY MISSED WORK.
EE STEPPED OFF THE BACK OF THE BOLTER, HIS RT. FOOT LANDED ON A PIECE OF COAL, TWISTING HIS RT. ANKLE.

EE WAS CROSSING TAILPIECE & CRIB WAS LAYING ON TOP OF IT. INSTEAD OF STEPPING OVER THE CRIB BLOCK & SECURING HIS HOLD, HE JUMPED ACROSS THE CRIBTO THE GROUND INJURING HIS KNEE. EE WAS SENT FOR MRI 3-3-01. DR HAS NOT READ REPORT AS OF 3-7-01.
EE WAS ON TOP OF MINER STARTED DOWN SIDE OF MINER WHEN HAND SLIPPED OFF & FELL TO GROUND INJURING RIGHT KNEE
EE WAS WALKING ON BELT THAT WAS DOWN, BELT WAS WET & MUDDY. HE WAS GOING TO CUT BELT INTO WITH HIS KNIFE. KNIFE WAS OPEN. EE SLIPPED & FELL ON IT.
EMPLOYEE SLIPPED AND FELL BRINGING ALL OF HIS WEIGHT DOWN ON HIS LEG BREAKING LARGE BONE IN LEG.
