EE was at the charging station on the supply road and parked the scoop in position to change out the batteries. As EE exited the scoop and traveled toward the batteries, EE slipped and fell striking the right side of face against the scoop frame.

EE was cleaning up around the 4C Header. EE lost footing and fell to the mine floor, ee felt pain in right shoulder/ankle. EE was treated for a mild strain, given a full release to regular occupation. On 7-16-18 ee seen doctor and was placed on restrictions that prevented ee from performing normal occupation. EE began missing time on 7-17-18.

Employee was walking on the surface of an underground mine to gather materials for EE's shift. As EE was walking, EE slipped on uneven surface that was slick from snow causing EE to fall to the ground. As EE fell EE caused a fracture to EE's left leg/ankle. EE began missing time on 01-16-18.

As ee was stepping up onto bolting platform, ee lost footing causing ee to scrape right shin on the frame of the step. EE continued working without restrictions performing normal occupation. On 01/17/18 ee was referred to physical therapy and placed on restrictions that prevented ee from performing normal occupation, ee began missing time on 01/17/18.

Employee was in the process of performing a pre shift on a Cat 777 end dump. We believe EE was climbing steps on the truck and fell from the lower level. Employee was found on the ground in front of the truck by an employee performing a pre shift inspection on the truck next to injured employee. At this time employee can not tell us what happened.

Employee was climbing down off of a pallet of concrete blocks, he had stepped down onto the ground with the left leg, however when he stepped down with his right leg he twisted his knee and felt a pop. When he put weight on his right leg he felt his knee pop twice.

The employee reported an alleged accident that he was on top of a diesel scoop and slipped and fell from the scoop, striking his right hip on the mine floor. On 12-13-12, he began missing work for what we view as a non-work related non-reportable lower back issue, which is not a work-place injury or accident.
