An employee was walking a belt line and EE slipped mid step, EE felt pain in EE's back.
An employee was jerking on ventilation curtain that was tangled in miner cable. The employee stated ee went to take a step after retrieving the curtain and felt pain in back that caused ee to drop to ee knees.
EE statement: I was tightening a joint of conduit with a 12" pipe wrench and my footing slipped causing my body to twist hurting my shoulder. Pain in my shoulder, Rt shoulder.
The Employee went to climb inside of the #6 rail ride, his foot got stuck in the mud and he fell into the ride catching his left hand on the SCSR box. He lacerated his palm and received sixteen stitches.
Employee stated he was walking by the lamp house and tripped he tried to catch himself and his knee gave out.
The Employee stated that he stepped in a hole firebossing the roadways on unit 006. When he stepped in the hole he felt his back twist.
The Employee stated he crossed thru a regulator and twisted his lower back.
The Employee stated that he stepped out of the scoop and his leg bent backwards twisting his right knee.
Employee stated that he he was tripped by a trailing cable and twisted his knee.
Employee was working on a dryer, the employee stated that he slipped and caught his foot and fell backwards injuring his tibia and fibula.
Employee was replacing a roller and his feet were in mud. When the employee turned he allegedly twisted back area.
Employee was standing on a curb outside of bathhouse near parking lot and another employee accidently stepped on his foot. Employee lost his balance and fell from curb fraturing his right ankle.
Employee slipped and fell, receiving slight fracture to his fibula.
Employee was picking up cable and his foot fell between a wooden bridge and the coal rib and he twisted his knee. The employee continued work until July 7, 2009, and received surgery on July 8 2009.
I was exiting the side of the mini-trac and caught my foot as I caught my foot I lost my balance and fell on my shoulder. No rules or regulation was broken. Bruise left shoulder.
Ee was on a ladder on uneven ground hanging bolter cable when a leg on the ladder bent sending him to ground and twisting his ankle. This injury did not involve any aspect of compliance with rules or regulations, job skills or attitude.
Working in canopy area of new underground mine, I was on a ladder working to take down a miner cable. A cable hook became unhooked from the canopy unexpectedly and the weight of the cable knocked me from the ladder. I fell backward onto my back & head.
While setting concrete forms, employee jumped aprox. 2.5 ft. into excavation to assist. He stated he hurt his back when he hit the ground.
EE WAS HAULING MEGA-BLOCKS TO THE SECTION WITH A SCOOP. HE HAD EXITED THE SCOOP & WAS WALKING BACK TO CHECK FOR CLEARANCE WHEN HE STEPPED ON UNEVEN GROUND & IN MUD & TWISTED HIS LEFT ANKLE. THIS INJURY DID NOT INVOLE ANY ASPECT OF COMPLIANCE WITH RULES OR REGULATIONS, JOB SKILLS OR ATTITUDE. Amendment: Returned to dr 2/20/06-has recurring pain. Back on restricted duty.
I and another employee was carrying a water pump (underground area) to set it. While doing this I stuck my left foot in the mud and twisted my left knee. This injury did not involve any aspect of compliance with rules or regulations, job skills or attitude.
EE WAS WALKING AROUND ROOF BOLT MACHINE, STEPPED ON UNEVEN GROUND (IN A HOLE) & TWISTED HIS LEFT ANKLE. THIS INJURY DID NOT INVOLVE ANY ASPECT OF MINE COMPLIANCE WITH RULES OR REGULATIONS,JOB SKILLS, ATTITUDE OR PROTECTIVE ITEMS.
EMPLOYEE WAS OPERATING A FLETCHER ROOF BOLTER. HE REACHED TO HIS LEFT TO GET A ROOF BOLT AND STEPPED ON A LUMP OF COAL AND HIS LEFT LEG AND KNEETWISTED CAUSING HIM TO FALL. HE CALLED TO THE OT HER ROOF BOLTER WHO GOT FURTHER HELP TO GET HIM OUT OF THE MINE. INJURY DID NOT INVOLVE ANY ASPECT OF COMPLIANCE WITH RULES OR REGULATIONS OR JOB SKILLS TRAINING.
WAS HANGING CABLE FOR ROOF BOLTING MACHINE. WAS ON LADDER. SLIPPED & FELL OFF LADER. WHILE FALLING, FELL AGAINST RIB OF COAL STRIKING BACK & SIDE AND TWISTED BACK.
EE WAS ON MINER WHEN HE SLIPPED AND CAUGHT HIS LEFT FOOT UNDER HIS BODY TWISTING HIS ANKLE. THERE WAS NO DAMAGE TO EQUIPMENT OR MINING OPERATION. THE INJURY DID NOT INVOLVE ANY ASPECT OF COMPL IANCE, JOB SKILLS, TRAINING OR ATTITUDE OR PROTECTIVE ITEMS.
