An employee was jerking on ventilation curtain that was tangled in miner cable. The employee stated ee went to take a step after retrieving the curtain and felt pain in back that caused ee to drop to ee knees.

The Employee went to climb inside of the #6 rail ride, his foot got stuck in the mud and he fell into the ride catching his left hand on the SCSR box. He lacerated his palm and received sixteen stitches.

Employee was picking up cable and his foot fell between a wooden bridge and the coal rib and he twisted his knee. The employee continued work until July 7, 2009, and received surgery on July 8 2009.

Ee was on a ladder on uneven ground hanging bolter cable when a leg on the ladder bent sending him to ground and twisting his ankle. This injury did not involve any aspect of compliance with rules or regulations, job skills or attitude.

Working in canopy area of new underground mine, I was on a ladder working to take down a miner cable. A cable hook became unhooked from the canopy unexpectedly and the weight of the cable knocked me from the ladder. I fell backward onto my back & head.

THIS INJURY DID NOT INVOLE ANY ASPECT OF COMPLIANCE WITH RULES OR REGULATIONS, JOB SKILLS OR ATTITUDE. Amendment: Returned to dr 2/20/06-has recurring pain. Back on restricted duty.

HE CALLED TO THE OT HER ROOF BOLTER WHO GOT FURTHER HELP TO GET HIM OUT OF THE MINE. INJURY DID NOT INVOLVE ANY ASPECT OF COMPLIANCE WITH RULES OR REGULATIONS OR JOB SKILLS TRAINING.
