Employee was walking an old beltline counting waterline joints when EE lost EE's footing and caught self by grabbing a belt chain causing a sharp pain in EE's shoulder. EE was first seen by a doctor on 11/06/2025 and continued working without restrictions until 02/04/2026 when EE was placed off work for an exploratory surgery to have EE's shoulder repaired.

Employee was loading a shuttle car in the first turn of 7 right. They went to take a step back and lost footing and fell backwards striking their head, neck and upper back against the shuttle car resulting in an acute fracture of the T6 vertebrae. Employee is expected to make a full recovery without surgery being required. They started missing work on 08/27/2025.

EE slipped, lost footing and hit chin on the handrail of the scissor lift resulting in a small laceration. (3) sutures were required to close the wound.

EE was getting out of EE's mantrip when EE felt something pop in right knee. EE was seen at OA on 6/10/24 and placed off work for a possible torn meniscus/pending an MRI.

EE was walking alongside the bolter in 6R and tripped over some gob in the face. EE caught right arm on a stack of roof bolt plates that was sitting on the bolter resulting in a laceration on the forearm. (6) sutures were required to close the wound.

EE stepped over some gob and twisted right knee resulting in a torn meniscus. EE started missing work on 03/12/2024.

While cleaning around the 1A head drive, employee slipped/fell and caught self on the guarding with EE's left arm, injuring EE's left shoulder. EE was seen by a doctor on 10/05/2023 and was diagnosed with a small tear in EE's left shoulder. EE continued working with a full release until EE's surgery date which was on 11/15/2023.

Employee was hanging tie wire and stepped off in a mudhole awkwardly, rolling right ankle. X-rays revealed a fracture in EE's right ankle. EE began missing work on 04/10/2023.

While marking up entry #3 on #2 unit, the section foreman stepped in a hole awkwardly and twisted left knee. They started missing work on 11/2/2022 and is being treated for a possibly torn meniscus.

While stepping on to the road grader, EE slipped and felt a sharp pain in EE's lower back. An MRI later revealed a bulged disc. EE continued working without restrictions until the date of EE's surgery. EE began missing work on 12/01/2022.

While stepping over a piece of metal, IE slipped and fell but caught themselves with left arm and shoulder. Causing strain/tear.

Sampler was monitoring the mechanical sampler on company property. EE went into the control room to get another bag when EE turned and tripped over EE's feet, fell out the open door and hit left arm on edge of concrete.

While walking to #1 entry, employee bent over to avoid a roof bolt hanging from the roof and slipped/tripped and landed on right shoulder. Dr. put off work with a strain until MRI.

As the employee was walking,tripped on some gob on the mine floor and twisted their ankle. This caused a tear in a ligament in the left ankle.

While cleaning walkway in beltline, bottoms were soft, EE's right foot got stuck in mud and when EE turned EE twisted right knee.

Employee tripped over a secured I-beam, causing EE to lose balance and land on hip and strained lower back.

The employee stepped off a supply trailer and strained hip.

The employee slipped while walking underground and twisted left knee. Began missing work on 9/14/2017.

The employee rolled ankle while walking.

The employee twisted left knee while walking to the roof bolter. Employee was given restrictions on 5/25/17 that do not allow employee to fulfill employee's normal work activity.

While walking to the shuttle car, the employee rolled ankle, causing a fracture.

The employee was entering the cab of shuttle car and twisted right knee.

The employee was picking up loose supplies and lost EE's footing. EE fell on EE's left shoulder causing a incomplete tear of EE's rotator cuff.

The employee was clearing loose material in the belt entry. He picked up a rock to place it on the belt and lost his footing. He lost his footing while placing the rock on the belt and his left middle finger got caught between the rock he was holding and the ground causing laceration. Sutures were required to close the wound

As employee was walking along the West side of the 405 conveyor, he stepped in a hole, causing his right ankle to turn abnormally, causing a fracture to his ankle. He was treated with an ankle boot and returned to work.

As the employee stepped off the slope car at the bottom of the shaft he sprained his ankle.

EE was in the process of setting/leveling unit #1 header. He tripped on a crib block causing him to fall. When he fell his left leg hit another crib block causing a laceration below his left knee. EE continued to work without pain. He seen his family Dr. on 5/1/14 due to pain in his left leg. The laceration had become infected. EE began missing work on 5/2/14

The employee stepped out of the dumping end of a shuttle car, causing a minor tear to his Medial Meniscus. He was able to perform his regular work duties until his surgery. He started missing work on 4/23/2014.

Employee slipped while walking down the 16 degree service slope. This caused him to sprain his left foot. He was caught by another person while he was falling and did not fall to the ground.

The employee was riding on golf cart and the golf cart became high centered on rock dust. The employee got off the golf cart to assess the situation and stepped in a hole and rolled his ankle.

The employee fell through ladder well, and to prevent the fall he caught himself. After the event, he reported pain in his shoulder and was taken off site for medical care.

EE while using a jackleg fainted and hit his head.

Employee loading & securing a package on an F-250 truck. As he attempted to get down off the truck bed, the hinged side board fell open. This caused him to fall onto his wrist causing a Sprain.

EE was walking a conveyor belt checking the remote line. As he was walking, he stepped on a piece of rock and lost his balance and fell onto his left hand, causing a fracture to his little finger.

EE stated that he stepped in a rut and twisted his right knee. EE did not report this accident to his Foreman or anyone else on this Friday. On Monday he called in and said he had car trouble, then later, when questioned, he said he hurt his knee. We feel this was a home accident and not work related.

EE was trying to pull down a piece of ventilation curtain. When he jumped up in a pull down motion. When he landed on his feet, his left ankle twisted and popped. He was transported to the hospital where an x-ray showed a fracture.

EE was standing on a 4' step ladder while plastering a brattice. As he was doing so, he slipped and fell off the ladder, striking his head on the brattice, causing a laceration that had to be closed with sutures.

Employee was walking down a couple of concrete steps that lead down to the entrance of a sample building. He missed a step causing him to fall, landing on his side on the steps. During the fall he received 2 fractured ribs. Employee said fall may have been avoided if he walked straight down instead of at angle. He also said it was dark outside.

While cleaning the 1st 48 belt drive, EE stepped over a cross member and lost his balance causing him to twist his right knee.

Employee was standing on a supply trailer stacking concrete bags on a pallet when his right leg fell in the hole where the wheel rail is located. He fell off the trailer on his back with his leg hung in the hole, hurting his right knee.

Employee was riding in the passenger seat of a golf cart, traveling from work area to stope bottom at end of shift. As the golf cart went around a corner turn, he left the golf cart before it stopped and twisted his left knee causing him to fall. This resulted in a fracture to his left knee cap area.

EMPLOYEE WAS WALKING NEXT TO A DOUBLE BOOM ROOF BOLTER WHEN HE TRIPPED & FELL CATCHING HIS LEFT HAND & FINGERS ON THE EDGE OF THE ROOF BOLTER CAUSING A LACERATION TO HIS FINGER.

Employee was changing out tail roller on unit 3 belt tail. He and his co-workers slipped with the roller and it caught his left thumb between the roller and frame of the tail piece. He received a laceration to his left thumb. He received seven sutures and returned to work.

Ee stepped across into a battery scoop bucket, his right foot slipped and he hyper extended his right knee. he sought medical attention and the mri showed a torn acl and torn meniscus cartialge. he is going to have surgery to repair his right knee.

Ee twisted his left ankle in the slope track while placing a small part in the slope car seating compartment. When his ankle turned over he fell and as a result of the fall he received a compound fracture to his lower left leg. He had surgery to repair the fracture on 7/28/04.

EMPLOYEE WAS IN THE PROCESS OF HELPING ANCHOR A BELT TAIL PIECE. HE WAS STANDING ON THE TAIL PIECE HOLDING A JACK PIPE WHEN THE JACK PIPE SLIPPED. HE LOST HIS BALANCE AND FELL OFF THE BELT TAI L. HE TWISTED HIS RIGHT KNEE WHEN HE HIT THE GROUND. EMPLOYEE CONTINUED TO WORK UNTIL MARCH 9TH. FIRST DAY MISSED DUE TO INJURY 3-10-04. KNEE SPRAIN.

EMPLOYEE WAS PICKING UP TRASH AND STEPPED IN A HOLE. HIS KNEE BUCKLED, TWISTED AND THEN POPPED. EMPLOYEE'S FIRST DAY MISSED WAS 5/8/03.

EE WAS CARRYING 2 SMALL PIECES OF METAL TO PLACE IN THE METAL RACK WHEN HE SLIPPED ON A PATCH OF ICE AND INJURED HIS KNEE. EE CONTINUED WORKING UNTIL HE UNDERWENT SURGERY AND STARTED MISSING W ORK ON 03-21-03.

EE WAS IN THE PROCESS OF WALKING DOWN THE BELT SLOPE TO APPLY CHLORIDE TO THE ICE THAT WAS ON THE FLOOR, WHEN HE SLIPPED ON THE ICE AND STRUCK HIS HEAD, ARM AND HAND CAUSING A HAIRLINE FRACTUR E TO HIS RIGHT HAND.

EMPLOYEE WAS STANDING ON A LADDER TRYING TO PLACE A SCREEN ONTO THE FLOOR IN THE PREP PLANT WHEN HE LOST HIS BALANCE AND FELL CAUSING HIM TO INJURE HIS KNEE BY BRUISING AND TWISTING IT.
