Employee was walking an old beltline counting waterline joints when EE lost EE's footing and caught self by grabbing a belt chain causing a sharp pain in EE's shoulder. EE was first seen by a doctor on 11/06/2025 and continued working without restrictions until 02/04/2026 when EE was placed off work for an exploratory surgery to have EE's shoulder repaired.

They went to take a step back and lost footing and fell backwards striking their head, neck and upper back against the shuttle car resulting in an acute fracture of the T6 vertebrae. Employee is expected to make a full recovery without surgery being required. They started missing work on 08/27/2025.
EE slipped, lost footing and hit chin on the handrail of the scissor lift resulting in a small laceration. (3) sutures were required to close the wound.
EE was getting out of EE's mantrip when EE felt something pop in right knee. EE was seen at OA on 6/10/24 and placed off work for a possible torn meniscus/pending an MRI.
EE was walking alongside the bolter in 6R and tripped over some gob in the face. EE caught right arm on a stack of roof bolt plates that was sitting on the bolter resulting in a laceration on the forearm. (6) sutures were required to close the wound.

While cleaning around the 1A head drive, employee slipped/fell and caught self on the guarding with EE's left arm, injuring EE's left shoulder. EE was seen by a doctor on 10/05/2023 and was diagnosed with a small tear in EE's left shoulder. EE continued working with a full release until EE's surgery date which was on 11/15/2023.
Employee was hanging tie wire and stepped off in a mudhole awkwardly, rolling right ankle. X-rays revealed a fracture in EE's right ankle. EE began missing work on 04/10/2023.
While marking up entry #3 on #2 unit, the section foreman stepped in a hole awkwardly and twisted left knee. They started missing work on 11/2/2022 and is being treated for a possibly torn meniscus.
While stepping on to the road grader, EE slipped and felt a sharp pain in EE's lower back. An MRI later revealed a bulged disc. EE continued working without restrictions until the date of EE's surgery. EE began missing work on 12/01/2022.
While stepping over a piece of metal, IE slipped and fell but caught themselves with left arm and shoulder. Causing strain/tear.
Sampler was monitoring the mechanical sampler on company property. EE went into the control room to get another bag when EE turned and tripped over EE's feet, fell out the open door and hit left arm on edge of concrete.

As employee was walking along the West side of the 405 conveyor, he stepped in a hole, causing his right ankle to turn abnormally, causing a fracture to his ankle. He was treated with an ankle boot and returned to work.

EE was walking a conveyor belt checking the remote line. As he was walking, he stepped on a piece of rock and lost his balance and fell onto his left hand, causing a fracture to his little finger.

During the fall he received 2 fractured ribs. Employee said fall may have been avoided if he walked straight down instead of at angle. He also said it was dark outside.

Employee was standing on a supply trailer stacking concrete bags on a pallet when his right leg fell in the hole where the wheel rail is located. He fell off the trailer on his back with his leg hung in the hole, hurting his right knee.

Employee was changing out tail roller on unit 3 belt tail. He and his co-workers slipped with the roller and it caught his left thumb between the roller and frame of the tail piece. He received a laceration to his left thumb. He received seven sutures and returned to work.

EE WAS IN THE PROCESS OF WALKING DOWN THE BELT SLOPE TO APPLY CHLORIDE TO THE ICE THAT WAS ON THE FLOOR, WHEN HE SLIPPED ON THE ICE AND STRUCK HIS HEAD, ARM AND HAND CAUSING A HAIRLINE FRACTUR E TO HIS RIGHT HAND.
EMPLOYEE WAS STANDING ON A LADDER TRYING TO PLACE A SCREEN ONTO THE FLOOR IN THE PREP PLANT WHEN HE LOST HIS BALANCE AND FELL CAUSING HIM TO INJURE HIS KNEE BY BRUISING AND TWISTING IT.
