IE was walking across the supply yard toward the warehouse storage building. While walking across the snow covered ground, the IE turned EE's body to speak with another employee located at the fuel pad. During this time, the IE stated EE took a step and slipped, falling to the ground on EE's left side, fracturing EE's left hip.

Injured was walking LOXC between entry #7 and #8, when they fell catching self with their arms causing shoulder injury.
The IE was descending stairs from the second floor in the Prep Plant. When the IE reached the landing between the 2nd and 1st floors, EE turned right with EE's left leg to start descending the next flight of stairs. As EE stepped down with EE's left leg, EE twisted the left knee.
On 01/02/23 a prep plant employee was descending a (3) step walkway over the Magnetite Chute. The IE slipped on the second step twisting the left knee. The injured employee continued to work until 08/28/23 where IE underwent surgery to repair a tear in the knee.
Employee stated employee has fallen several times but cannot remember a specific date, but stated cannot lift arm. Employee was taken off work on 09/10/2021 for surgery of a torn rotator cuff of the right shoulder.
IE stated was stepping up on a ladder to hang a line curtain. As IE reached the second rung of a 4' ladder, IE reached up to grab the hog panel to stabilize self, the ladder began to tip, causing IE to fall to the ground. IE began to complain of back pain. IE was taken to a medical facility for observation.
IE stated that EE was examining the belt line. IE slipped and began to fall forward. When EE tried to catch self from falling forward, EE grabbed the belt chain with EE's right hand and hog panel with EE's left hand and then felt pain in both shoulders. The IE had surgery on 5/15/2020 making this reportable.
IE was packing 60" top belt rollers to supply trailer when IE tripped over a stick of 6" waterline. IE fell into the supply trailer striking IE's left knee and thigh. IE received a 1" laceration on IE's inner left thigh, which required 6 sutures to close the laceration.
Employee was in the process of moving the continuous miner cable off of a getman and lost balance causing them to fall into the canopy of the getman.
The IE was getting out of a scoop when left foot got caught. This caused the IE to put more pressure on right foot and twisting it. IE stated heard 2 pops in ankle and fell to the ground and was in pain.
IE stated picked up a bundle of bolts, began to turn towards the hauler, feet got tangled up on a pallet of supplies and started to fall. During the fall, IE tossed the bolts and tried to catch balance. Left arm/shoulder twisted and rolled in a backward motion. IE was brought out of the mine, taken medical facility & arm placed in a sling, with no use of left arm.
IE stepped in a rut while roof bolting. IE felt pain/warmth after incident. IE was treated at ER and released. IE was treated at OISI on 9/25/18 and released. An MRI was performed on 10/12/18 and revealed a tear of medial meniscus. IE followed up OISO on 10/22/18 at which time a non-weight bearing restriction was applied becoming lost time.
A Contract employee was building a block wall at 30 xc on the 4th SW right side. EE was stacking block on the wall, when ee picked up a block and started walking toward the wall, ee tripped on another block lying on the ground. EE went down on left knee landing on the side of the block, causing a sprain/ contusion. The employee was taken off work.
A contract employee had walked into the shower and had been in there approximately 3 - 4 minutes. employee just finished washing hair, turned around to grab body wash and as they turned, they slipped, their feet came out from under them causing them to land on their head causing a laceration that took 10 staples to close the wound.
IE was standing checking rail car doors to make sure they were latched. IE alleges that right knee twisted/hyperextended as IE was standing there. IE did not report this incident until 06/26/2017. IE continued to work up until 08/10/2017 when IE had right knee surgery. Surgery was arthroscopic partial medical meniscectomy and debridement of plica.
On 6/13/17 the EE stated that they stepped in mud while walking. When they pulled their foot from the mud, the knee began to hurt. The EE came in on 6/19/17 and stated that the knee was not getting any better and wanted to go see the Dr. The EE was taken off work on 6/19/17 pending a MRI. Results of the MRI revealed a rupture of a Baker's Cyst.
Employee slipped on rock in step while dismounting dozer and fell to ground.
EE was walking between the Continuous Miner and the rib, slipped/tripped on some loose mine floor material and began to fall. EE reached out with arms to try to catch themselves on the side of the machine when EE felt a sharp pain in upper left shoulder. EE was transported to the hospital and treated for a dislocation and MRI results show a torn labrum.
Employee said on 1/16/2017 EE fell as EE stepped off the concrete pad near the take up. At the time of the incident EE said didn't think much of it and continued to work. On 1/17/2017 EE noticed pain. Employee began missing work on 1/23/2017.
IE was walking in the Main South Intake and tripped over a rock. When IE put Hand down to catch self, smashed left pinky. IE continued walking to the 1st E. Seals then stopped to check hand and seen finger was split open.
IE was walking from entry 2 to 3 towards the battery scoop ee was operating. As ee was walking ee stepped in a hole and twisted knee as ee landed. IE continued to work with no restrictions until 8/18/16 when ee had surgery to repair the ACL.
IE was carrying tools back to tool hole from working on the miner which had already been fixed. EE had tools in both arms and a small shovel under EE's arm when EE got tripped up on EE's feet and fell to the ground on EE's knees. Causing EE's left knee to swell. IE continued to work but pain and swelling progressed. Employee did not report on next scheduled shift.
3/23/16 IE said ee tripped over a CM cable and twisted knee. IE finished the shift, was examined the next day and returned to full duty. Follow up MRI was done 6/2/16. IE was returned to full duty scheduled for orthoscopic surgery 6/21/16 for loose body removal. Post Op. notes revealed chondromalacia deterioration grade 2&3 but also an acute traumatic meniscus tear.
Employee said EE was sitting on the battery end of a scoop. While dismounting EE said their foot slipped off the steering jack. EE said they fell and caught themselves with their left arm injuring EE's left shoulder. Employee began missing work on 3/21/2016.
As the IE stepped down out of the feeder with his left leg, he also struck the back of his hard hat on a chain hanger. The combination of these two incidents caused the IE to lose his balance, causing him to strike his right knee on the hopper end of the feeder.
IE was walking Unit 1 belt line. He stumbled and lost his balance then fell to the ground catching himself with his right hand. He continued to work up until he had right shoulder surgery on October 27, 2015 to repair a SLAP tear.
Employee alleges that he injured his back while walking the main East belt during his on-shift examination. Operator has been waiting on MRI results. MRI was conducted on 8/06/2015 and results were received on 08/12/2015. MRI indicates a small disc protrusion. Employee remains off work.
The IE stepped over a 3/4" hose and it caught his left foot causing him to trip and twisting his left knee

IE was descending the stairs from the cab of a 988G end loader. IE had his left hand on the hand rail he misjudged the last step, lost his balance, and began to fall backwards. He was able to briefly make contact with the handrail at the rear of the machine, which changed the direction of his fall, causing him to fall to the ground on his right shoulder.
IE was walking down the belt line and as the IE was passing the booster pump, he stepped over the main waterline to avoid the hoses from the booster pump, when his right foot came down on a chunk of coal. His leg kicked out which was going to cause him to fall. He reached up with his left arm and grabbed the belt chain to keep himself from falling and injured his left arm.
The IE was setting the continuous miner to the other side of a cut, when he stepped in a rut twisting his left knee.
Employee was exiting a storage building at an abandoned mine site. When he exited the door he slipped on a layer of ice. He dislocated his shoulder. The shoulder was reset by the hospital physician. He was released to return to work. He now has been given work restrictions and will seek further care. Is scheduled for an MRI on April 3.
Employee was operating a Continuous Miner and while standing stationary he had to turn and twist his body to see the approaching Ram Cars. He felt pain in his right knee. He had right knee arthroscopy and partial medial meniscectomy on May 13, 2014.
The operator of the Hitachi EX700 excavator slipped while walking on the track. He fell 43 inches to ground resulting in torn ligaments in his left knee. His first shift missed was on 01/10/2014.
Employee was getting out of trash dumpster when he missed a ledge on the dumpster and twisted his right ankle. Restricted duty and wear a brace on right ankle.
Injured employee was exiting the cab of a diesel scoop when he twisted his right knee on the mine floor. He continued to work up until September 13th when he was taken off work for a torn meniscus in his right knee.
The employee stated he had some trash on a wooden pallet in the bucket of the battery scoop to take outby to unload. He was standing on the pallet picking up trash and tossing in the dumpster. The pallet broke, and when this happened the employee fell against the side of the scoop bucket striking his hand on the inside of the bucket causing a laceration that required sutures.
The employee stated while installing his middle roof bolt in Entry #5 a rock fell inby supported top startling him. While trying to move away his left foot was caught between some rocks on the mine floor. While moving his left foot twisted spraining his ankle. He stated to his supervisor he felt a burning in his left ankle. He did not report to work on his scheduled Monday shift.
The injured employee was removing material from the third floor of the Prep plant, a protrusion from a piece of steel he was lowering to the ground caught his jacket and pulled him between the bars out of the hoist entryway, to the ground below. He was transported to Deaconess Hospital with injuries to his right ankle and heel.
Employee was on top of a de-energized continuous miner. He stepped up on a flare board on the miner tail and slipped off striking his left leg on the top of the flare board. He received a laceration to his left shin that required 6 sutures to close.
The injured was walking next to the #38 Roof Bolter preparing to spot bolts on a corner. A piece of roof rock measuring 48" X 19" X 9" thick fell from the mine roof causing the injured to fall to the ground. He suffered a compound fracture to his left middle finger. The incident occurred on MMU 001-0, XC #152, between #7 and #8 entries.
The employee stated that the service maintenance ride was down, so he was walking with a can of hydraulic oil to take to a coal hauler to fill it up, when he stepped in a hole and twisted his knee. The IE continued to work, then on 1/20/12 had surgery to repair a torn meniscus.
The injured EE was walking between entry # 1 & entry #2. He was going to the roof bolting machine entry. He stumbled, twisted an ankle and fell. He felt something pop in his back when he hit the ground. He was transported out of the mine at 8:40 p.m. He received further medical attention thereafter. He remains off work.
Employee was washing a refuse feed box in the prep plant. He stepped from a concrete floor down to a metal floor approximately 3 inches lower. He slipped and fell striking his left elbow on the floor. He continued to work up until September 1 when he was taken off for surgery to repair a tendon in his left elbow.
Employee was walking between a coal rib and his battery ram car. He slipped on a wind row of loose fire clay and twisted his left knee and ankle. He fell to the floor and rolled his left ankle resulting in a fracture to his left ankle.
The injured tripped over a piece of 6X6 timber causing him to fall over the top of it contacting his right leg. The incident occurred while training an employee to change batteries on Unit #1. The incident caused a fracture to the right fibula just above the ankle.
The employee was preparing to set a pump following a 2" rain. He was approximately 40 ft. from the highwall and there was a protective catch berm between him and the wall. He said he heard something fall from the wall so he ran. The employee fell and received a laceration to the head that required 6 sutures.
Injured employee was getting off the coal hauler and fell landing on his right hand breaking it in 2 places.
