Showing all 29Employee was getting out of scoop deck. He had his right foot on the ground and was removing his left leg to plant on the ground. When he twisted his right knee popped. He continued working until 6/11/2015 when he had surgery on his right knee.
Walking through door on bottom, fell through door and sprained left foot/ ankle.
EE was washing the road grader when his feet became entangled in the wash down hose causing him to trip and fall on his right knee. This resulted in ee not being able to return to work on his next scheduled shift.

Employee was walking into 3D header and stepped hole twisting his right knee, on 10/15/2013. He went and had x-rays completed on 10/15/2013 and had negative findings. He then saw a orthopedic surgeon on 10/31/2013, which he placed him off of work until 11/04/2013.
