He came back to work and worked two days then went back to hospital and was put off work with a strain. We are disputing his comp claim due to the fact he told other employees that he hurt his back off of the job at home.
While doing his normal job position, employee started having symptoms of lower back pain. After extensive testing the results show he is suffering from the occupational illness of degenerative discs. Per 2/5 document 220102230004, EE was doing walk around his truck, when he slipped and fell on flat ground.
Employee jumped off shovel and sprained ankle while landing on a rock.
Employee was welding while working off of scaffolding at a height of 6 feet 3 inches. He fell between two scaffold boards and landed on his knees, resulting in a back injury.
Employee was putting the hose back on the pulley of the repump truck. After finishing he went to climb down the ladder, when he twisted he felt a burn in back. He finished the shift and decided to seek medical attention on 2/4/10.
Member was walking up a snow covered bank when he slipped and fell. His head struck the ground resulting in a laceration requiring two stitches.
Employee was exiting the 495 shovel when the ladder shifted and he fell approx. 3 feet to the ground. His left knee struck a rock and caused a small laceration. He was taken to the Boone Memorial Hospital where he received 8 stitches to his knee.

He was taken to the hospital where he was referred to a specialist for an MRI. He was returned back to work full duty. On 8/7/09, he went for the MRI reading and found a tear in his meniscus and was taken off work.
Employee was walking on job site with welding equipment in hand and fell on a loose rock. When he fell, he hit his head & neck on a welded support on a service truck. Initially medical evaluation came back negative and he was referred to a specialist. On 6/2/2009 specialist prescribed physical therapy & meds making the injury reportable.
Cleaning up & putting tools away in muddy conditions. EE lost footing & fell off truck landing on right hip and elbow.
An employee was attempting to change out a compressed gas cylinder on the Boom Service Truck. As the employee was attempting to manually lift the bottle he lost his footing in the mud and strained his right shoulder.
EE states that he was climbing into the ex200 excavator, when his right foot slipped off of the first step(approx 18 inches from the ground) and landed on his knee.
Employee slipped while exiting truck and injured back on the 22nd. He did NOT seek medical attention or miss work. On the 24th, his vehicle slid into berm in snow, reaggravating injury from the 22nd. He did not seek medical attention until the 28th, at which time he was off until 2/2/08.
Following a fuel delivery to customer tanks, EE was returning his 3" x 20' hose to the storage tube when he slipped on the large stone drive surface. In a attempt to break his fall, he stuck out his arm and felt a "pop" in his right shoulder. Reported to Progress Coal.

Employee was checking holes where he was drilling when he slipped on a rock and fell to his right, falling on an old broken steel pipe. Upon falling he cut the outside right side of his knee.
When starting to fuel, the nozzle separated from the hose. He jumped back to avoid the whipping hose & fell in the shot rock. He got fuel in his eyes & on his clothes. He struck his leg, knee, buttocks, hip, neck & head against the rock. He retrieved the hose & went to the maintenance area where he flushed his eyes, changed shirt, repaired hose & continued his shift until 7 am.
Employee was returning a tray of 630 amp fuses to the parts trailer, tripped over the corner of a box of parts on the floor. He fell into a shelf striking both arms against the shelf. Sustained injuries to both arms.
Departing the porta-john, EE stepped on an uneven surface, twisting his ankle. Investigation continuing, this injury is contested as having occurred off site.

On 10-7-05, he ceased coming to work and did not Mgt aware the he was seeking another dr opinion. The second dr removal him from work and place dhim on the port, mgt was not make aware of 2nd dr.

EMPLOYEE WAS WALKING UP THE STAIRS WITH A BAG OF NUT & WASHERS, WHEN HIS METATARSAL BOOTS GOT CAUGHT ON A PIECE OF 3/4" CONDUIT STICKING UP 4"-6" ABOVE THE WALKING SURFACE. HE TRIPPED AND FELL , CATCHING HIMSELF IN A FORWARD FALL WITH BOTH HANDS, TWISTING HIS BACK.

EE HAD CLIMBED ONTO A D-11 TRACTOR TO FILL IT WITH HYDRAULIC FLUID. WHEN HE REACHED OUT TO REMOVE THE HYDRAULIC TANK CAP HE LOST HIS FOOTING AND FELL APPROXIMATELY 8' TO THE GROUND LANDING ON HIS RIGHT SIDE. HE POSSIBLEY SLIPPED ON SOME SPILLED HYDRAULIC FLUID.
EE WAS STANDING ON THE GROUND, PULLING THE WASTE OIL HOSE OFF THE REEL OF THE GREASE TRUCK, TO PERFORM SERVICE ON A ROCK TRUCK. AS HE WAS PULLING, THE END OF THE HOSE CAME OFF CAUSING HIM TO L OSE HIS BALANCE & FALL TO THE GROUND. HE ATTEMPTED TO BREAK HIS FALL WITH HIS LEFT ARM, LANDING ON HIS ARM INJURING HIS WRIST.
CROSSING CATWALK ON LAUNCH, SLIPPED AND SHIN STRUCK CATWALK, CUT SHIN.

EE WAS WALKING ACROSS WALKWAY OF MINER LAUNCH PAD OF HIGHWALL MINER. HE ALLEGED HIS FOOT SLIPPED & HIS KNEE WAS LACERATED WHEN IT STRUCK THE METAL WALKWAY. LACERATION WAS APPROX 1/4 " LONG.
EE WAS STEPPING OFF A SET OF STEPS FROM LAUNCH PAD OF THE HIGHWALL MINER. HE STEPPED IN A HOLE ON PIT FLOOR. HE ALLEGED THAT HE TWISTED AND STRAINED HIS HIP. HE STARTED MISSING WORK DUE TO THIS ACCIDENT ON 5-24-02.
FUELER APPROACHING DOZER TO FUEL. WALKING THROUGH MUD SLIPPED ON A PIECE OF MUD COVER SLATE AND FELL. ATTEMPTING TO CHECK FALL EXTENDED RIGHT ARM AND HYPEREXTENDED WRIST FINGERS. EE LATE IN R EPORTING INJURY AS "ON THE JOB".
EE WENT UPSTAIRS OF HIGHWALL MINER TO SET UP CAR BREAKER. HE ALLEGED WHILE WALKING DOWN STEPS HIS ANKLE WAS SPRAINED WHEN HIS FOOT GAVE OUT.
EE WAS GREASING A WIDE BODY TRUCK. AS HE MOVED BETWEEN THE TIRES HIS FOOT CAME IN CONTACT WITH THE DUMP CHAIN CAUSING HIM TO FALL TO THE GROUND. HE LANDED ON HIS LEFT SIDE STRIKING HIS FACE, B ACK, LEFT LEG AND LEFT ARM ON THE GROUND.
EE WAS CLIMBING STAIRS TO LOADING RACK WHEN HE SLIPPED AND FELL. HIS GLOVED HAND (RT) SLIPPED FROM THE RAILING AND IN ATTEMPTING TO CATCH/BREAK THE FALL HIS LEFT WRIST BENT CAUSING A SPRAIN.
EE WAS CLIMBING DOWN FROM THE BACK OF MECHANICS TRUCK. HE TRIPPED OVER A LOOSE PART & FELL TO GROUND.

SMALL ROCKS AND DUST BEGAN F ALLING ONT HE CAB AND HE GOT OUT OF THE SEAT TO MOVE AWAY FROM THE FRONT GLASS. AS HE WAS MOVING TOWARD THE BACK A ROCK MEASURING 8 FEET LONG 3 FEET TALL AND ABOUT 4 FEET WIDE FELL FROM THE H
EE WAS CLIMBING OUT OF TRUCK. HE LOST HIS FOOTING AND JUMPED TO THE GROUND.
EE WAS WORKING ON TOP OF SHOVEL BUCKET WHEN THE FORKS/MAST CAME OFF THE FORKLIFT AND STRUCK EE IN THE BACK KNOCKING HIM OFF THE BUCKET ONTO THE GROUND.
EE WAS WORKING ON A 16G MOTOR GRADER, SLIPPED AND FELL OFF OF TIRE OF MACHINE LANDING ON BACK.
EE WAS CLIMBING DOWN MACHINE AND CARRYING A COUPLE OF WRENCHES. HE WAS ONLY USING ONE HAND, EE LOST BALANCE AND FELL AND HIT OUTRIGGER. BRUISED RIBS AND CUT BACK.
