Todd Pisciotti was stepping over the Cable Reel tow cable behind the P&H Shovel and fell onto the ground dislocating his left ring finger. He was taken to the hospital to have it re-aligned causing a Medical Treatment Case. He returned to work on his next scheduled shift. (Misc Mail 5/30/08 #1-he went out for surgery on 5/15/08 and was placed on restricted duty 5/16/08)

Showing all 16EE was carrying a hydraulic cylinder over his right shoulder. He was on his way to the next floor. As he apporached the first step, he slipped on ice and the cylinder landed on the metal floor, crushing the tip of his right middle finger between the cylinder and the floor.
Employee attempted to climb up the ladder of her haul truck, when her left leg slipped off of the first step of the ladder and she slid through to her knee. She felt pain in her left knee. She was seen by a doctor and given pain medication the next day resulting in a medical treatment case.
Employee stepped on the battery box and slipped straining his right knee on the 4008 drill. He continued to work and the pain did not ever go away. On November 28th he had orthoscopic surgery and will miss work.
Employee fell onto ice while attempting to move shovel cable in pit #1. Employee dislocated right shoulder and strained right ribs & neck. Employee returned to work on next scheduled shift.
Employee stepped from DRAGLINE roof into manlift & caught his right foot on a pedal, bending his foot causing pain to his ankle. He was treated & released to return to work. Transferred to another job. 3rd day in complained of pain to ankle.Taken to clinic 3/25/06, referred to ortho who found that he had a broken ankle. Taken off work for 3 days & released to modified duty.
EE SLIPPED WHILE EXITING A CREW BUS AND FELL, DISLOCATING HIS RT.SHOULDER.
EE WAS WALKING ACROSS A GRAVEL PAKING AREA, WHEN SHE TRIPPED AND FELL. SHE HIT THE GROUND AND DROVE HER GLASSES INTO THE BRIDGE OF HER NOSE, CAUSING A LACERATION THAT REQUIRED 3 STITCHES.
EMPLOYEE'S FOOT BECAME ENTANGLED IN HIS LANYARD FROM THE FALL PROTECTION HARNESS HE WAS WEARING. AS HE DESCENDED A LADDER, HE FELL APPROXIMATELY 2' TO THE FLOOR, LANDING ON HIS LEFT ANKLE.
SLIPPED AND FELL STRIPPING THE FLOOR.
EE SLIPPED AND CAUGHT HIMSELF AND STRAINED HIS LEFT ARM.
THE EE WAS CHECKING FLUID LEVELS ON A TRACK DOZER AND SLIPPED WHILE CLIMBING DOWN. THIS RESULTED IN AN INJURY TO HIS LEFT KNEE.
THE EE WAS HELPING DRAG A CHOKER CABLE UP A PILE OF CRUSHED SCORIA ROCK. WHEN HE REACHED THE TOP HIS KNEE STARTED POPPING. THE NEXT DAY, HE WOULD NOT BEND THE KNEE AND WAS DIAGNOSED WITH A CAR TILAGE TEAR THAT EVENTUALLY REQUIRED SURGERY.
EE SLIPPED ON ICE WHILE CARRYING A GREASE PUMP ON HIS SHOULDER. HE FELL AND THE PUMP LANDED ON HIS LEFT RING FINGER, FRACTURING IT.
EMPLOYEE FELL APPROX 5 FT FROM A MANLIFT.
EE WAS CLEANING THE REAR CAB WINDOW OF A CAT SCRAPER AND FELL FROM THE FENDER TO THE GROUND, RESULTING IN A FRACTURE TO THE LEFT TIBIA JUST ABOVE THE ANKLE.
