Employee was exiting the porta potty at the mine site headed back to her truck that was parked 15 feet away, when she fell forward hitting the ground. She was taken to the Hospital where x-rays showed her right wrist to be broken. She returned that day on light duty.

Employee was initially injured on 10/5/13 at the 23rd West drive area when he was standing on a cinder block to operate a come-a-long and his foot slipped off of the block thus spraining his left ankle. EE was examined and released by Dr. on this date. On 10/14/13 he returned to the doctor and was given a modified work release.

As the employee exited the forklift on the surface, he slipped on the ice and twisted his left knee, EE was taken to the hospital and examined and released to return to work. EE had a follow-up visit on 2/20/2013 and a MRI was conducted on 2/26/2013. On 2/27/13 the EE was referred to Orthopedic surgeon and instructed to be off from work until after surgery.

While retrieving vent tubes from the face area of the #1 entry in 21st West, the employee twisted his left ankle. EE was examined by a physician and the results of the x-ray were negative and the EE was released to return to work. The next day he return to the physician and was given a work release excusing him from work for that day.

While conducting an on shift examination in the tailgate area of the longwall in 17th East, The examiner lost his balance while traveling thru an area where the shields had been pulled in and when he reached down with his right hand to catch himself, he felt a pop in his right wrist. X-rays indicated a hairline fracture to a small bone in the back of the wrist.

Employee was originally injured on 6/26/12, when he tripped over a relay bar on the longwall face in 15th East and jammed his left shoulder into the shield leg. EE was taken to the local hospital for observation and released to return to work. On 9/24/2012, the employee went in for surgery on his shoulder, therefore becoming a lost time injury.

Employee was attempting to carry a bad belt roller from the belt line into the intake, as he attempted to go thru the walk thru mandoor in the stopping, he tripped over his own feet and hit his left shoulder on the frame work of the mandoor.

Employee was walking down the stairs out side of the con spec building and as she went to step off from the last step of the stairs, onto the ground, her foot slipped and she stained her right hip as she struggled to regain her balance.

Employee was tightening the lug nuts on the section battery scoop by using a 1/2 inch drive ratchet with a cheater pipe when the lug nut broke off and the employee fell to the ground thus spraining his right ankle.

Employee was changing out bad rollers along the #3 min line belt. EE was dragging a roller along the belt line while walking backwards and he tripped over another roller which had just been removed from the belt line, resulting in the emplyoee spraining his right ankle.

The employee was standing on the relay bar of shield #50 washing the shear, when his foot slipped off of the relay bar and he fell to the ground and hit his head on the toe of the #50 shield. 9 stitches were required to close the wound to the back of the head.

Employee was using a weed eater to cut down weeds when he lost his footing and as he almost fell, he reached out for a jersy barrier to catch his fall. As a result he injured his left shoulder. EE continued working until he went in for surgery on 9/15/2011.

Employee was attempting to place a roof mat onto the ATRS of the roof bolter and stepped too far, and as he stepped off of the cat walk of the roof bolter, he caught himself with outstretched arm on top of the canopy and ATRS frame work resulting in a strain of the left rotator cuff.

As EE was stepping off of the roof bolter, his right hand slipped on the wet surface of the machine resulting in a dislocated #5 finger on his right hand.

As EE was getting out of a mantrip at x-cut 65 in the left mains, his left foot got stuck in the mud and he strained his left knee as he lifted his leg. EE continued to work until 6/16/2010, at which time he had surgery on his left knee.

EE was working approximatly 60 feet inby the #3 portal when he slipped on some ice and fell to the ground, landing on his belt wearable CSE 100 self rescuer, resulting in the fracturing of 2 ribs on his right side.

EE was walking across the road at x-cut 54 in the right mains when his foot slipped out from underneath him and as he fell to the ground he injured his left shoulder. EE continued to work and visit his doctor until 11/17/09 at which time he underwent surgery on his shoulder.

EE was carrying a timber from a timber pile to a pick-up truck, when he tripped over another timber, resulting in a twisted ankle.

EE tripped over a cable on the shop floor. EE was treated at the ER for a back strain and was released to return to work with no restrictions. On 2/2/09 EE went back to the Dr. and was treated for a herniated disk in his lower back.

EE was standing on the relay bar of shied #3, washing the shields when his foot slipped off of the relay bar and as he fell, he landed on his right leg which had folded up under him resulting in a stained knee.

As EE was walking around the battery scoop, he stepped on a lump of coal and fell to the ground. As he fell he stuck out his left hand to catch his fall thus injuring his left wrist and hand.

EE was attempting to bend a 6 foot roof bolt so that he could install it. He was using the scissor area of the machine to hold the bolt and as he pushed on the bolt to bend it, the bolt slipped and the EE fell and struck his elbow on the drill box and hit his knee on the ground. EE work the next 2 days and went to the doctor on 4/24/2008.

While attempting to lower the headroller structure, the belt chain that was used to hang the come-a-long to the monkey face broke and the EE lost his balance and fell against the stucture of the headroller frame.

EE was walking down the belt entry along the take-up area when he slipped on a piece of mine belting which was being used to cover a tripping hazard. The belting was covered with rock dust and was wet.

EE sprained his right ankle as he stepped off of the walkway of the Fletcher double boom bolter.

EE as walking along the belt line on a 12%-15% grade when his foot slipped out from under him and he fell to the ground landing on his right hip. the rock floor was wet and slippery.

EE was changing bits on the miner. He was standing on the bits in an attempt to rotate the head when his foot slipped off of the head and he hit his elbow on the bits.

Carrying square sets, tripped (slipped) & fell on wet flooring, strained his lower back.

Employee was operating a loader. He went to jump off and the machine started rolling backwards. Employee landed on some crib blocks on his left knee.

EE was walking up the belt line when he slipped and hurt his right shoulder trying to stop his fall.

EE WAS GETTING VENT TUBES SO HE COULD ADVANCE VENTILATION IN THE FACE WHEN HE TRIPPED ON A LUMP OF COAL, THUS HURTING HIS BACK.

A bundle of roof screens were leaned up against the rib. ee was retreving one screen when the bumple fell over and knocked ee down to the ground resulting in a broken wrist on his left hand.

As ee stepped off the walkway of the roof bolter, when he stepped on the mine floor his foot slipped, he felt a sharp pain in his lower back.

Employee was removing vent tubes when he twisted his ankle.

Was climbing over pan line, his foot got tangled in some hoses and he fell in pan and landed on a flight on his back.

EE WAS CLIMBING OUT OF THE CONVEYOR WHEN HE SLIPPED AND FELL TO THE GROUND. HE CAUGHT HIS FALL WITH HIS ARMS. HIS LEFT ARM WAS DISLOCATED AT THE SHOULDER.

THIS EE WAS TIGHTENING A BOLT ON AN ACCESS DOOR PROVIDED FOR THE CRUSHER WHEN HE FELL THROUGH A HOLE IN THE FLOOR CREATED WHEN A SECTION OF FLOORING WAS REMOVED TO SHOVEL COAL INTO THE LOADER BUCKET. HE FELL INTO THE LOADER BUCKET.

THE EE WAS GETTING OUT OF THE GREH 1 SKIP LOADER AND HE STEPPED ON THE LOAD HE WAS CARRING ONT HE FORLES. HIS FOOT SLIPPED ON A RESIN BOX AND HE FELL STRIKING HIS BACK ONT HE FRAME WORK FOR TH E FORKS.

EMPLOYEE WAS WALKING ON WET SLICK FLOOR, SLIPPED ON UNEVEN SURFACE AND TWISTED HIS KNEE. EMPLOYEE FILED ACCIDENT ON 2-19-01, BUT DID NOT SEEK MEDICAL TREATMENT UNTIL 3-13-01. DOCTOR DIAGNOSED POSSIBLE DAMAGE TO KNEE THAT MAY REQUIRE SURGERYEMPLOYEE HAS NOT MISSED WORK AT THIS TIME.

WHILE HANGING THE ROOF BOLTER CABLE, STEPPED ON PIECE OF COAL TWISTING RIGHT ANKLE AND KNEE. SAW DOCTOR FOR KNEE PAIN 9/6/00 DIAGNOSED CARTILEGE TEAR IN KNEE (RT). SCHEDULED FOR SURGERY 9/13/0 0.

WHILE PUTTING ROOF BOLTS IN A NEW OVER CAST. THE INDIVIDUAL STOOD UP ON THE SIDE. OF THE ROOF BOOLTER STORAGE DOCK. HE LOST HIS BALANCE AND FELL ONTO THE OPPOSITE SIDE OF THE ROOF BOLTER'S STO RAGE DECK. THIS BRUISED HIS RIGHT RIBS REQUIRINGA RIB BELT AND MEDICATION.
