Showing all 42Operator was dismounting a 785B End Dump, off a ladder when incident occurred. Operator missed the last rung, fell backwards to the ground. Put out left hand to break fall, fell on left hand. Fracturing left distal radius.
When troubling shooting a limit switch, on a load out chute and is 25 feet above ground level. The electrician was utilizing mobile scaffolding to access the limit switch. The electrician adjusted the switch and raise the chute. When the switch was activated, the chute began to rise and struck the scaffolding. EE fell, struck the scaffolding and fell on concrete floor.
Employee strained his back dismounting a dozer.
Employee was resetting calibration limits on the 8750-1 dragline drum for a re-socket. When done, he was walking back to his work truck and twisted his left ankle on the ground transition from compact to soft and rock material beneath him. He refused Mine clinic treatment but went to the hospital later that night, where they diagnosed a sprained ankle.
Injured tripped over a 4" x 4" piece of cribbing in the motor shop while moving dragline house fans with the overhead crane resulting in a fracture to the right shoulder.
Employee slipped on gravel while loading cable on to a reel truck at the N-9 Cable Yard twisting his ankle and resulting in a left knee strain.
Mechanic was observing another mechanic removing final drive of a dozer. Tool: bracket fell and mechanic was trying to get out of the way, trip and fell on his face. Causing a 1/2 inch laceration on his right forehead, which required three stitches. Mechanic was treated and released back to work.
Preparation laborer was cleaning with a water hose when his leg became wrapped by the hose. He took a step forward he lost his balance and fell backward striking the back of his skull against a concrete floor.
The injured was maneuvering a belly pan to install it on a dozer when he slipped on a piece of ice. He fell backwards landing on his left shoulder and injuring his lower back.
A dragline operator was observing a winch cable being pulled by a nylon rope when the nylon rope broke. The elastic force of the nylon rope struck him from the left side knocking him to the ground.
EE fell while walking on grade (level) towards the back of a piece of equipment and injured his right leg. (fracture)
Serviceman fell backwards from his truck landing on his back striking his head against the ground.
The oiler tripped over a metal cover while walking inside the dragline, striking his right shin against the edge of the metal cover.
Employee went to start service truck and let warm up, there was a patch of ice that was covered by snow, he walked on this area and slipped, twisting left knee.
The supply-man was pulling on a cardboard box of parts on the truck flatbed when the box tore at the sides, causing him to fall backwards against the truck head-rack. He struck head against a channel iron bracket lacerating the base of his skull. The incident occurred when he was in the process of unloading hoisting parts onto the dragline.
The dozer operator was standing on the cab platform lifting a nylon sling, when he lost his balance and decided to jump clear of the tracks and landed feet first on the ground. He injured his left forearm when it hit the side blade arm when he jumped off his dozer.
The welder was walking on a catwalk when he sprained his left ankle causing him to fall through an opening at the end of the walkway. He fell onto the next level injuring his ankle when he landed on metal surface.
The welder placed power cable on dragline tub cable-hook and stepped back, when his left foot slipped underneath tub that was coming back down during deadhead of machine. Left foot-toes were pinched between the ground surface and the tub.
The injured was dismounting a dragline when the portable ladder gave away from underneath him. He fell sideways onto the dragline shoe sustaining injuries to his face and bruising his right shoulder.
EE was picking up trash and filling into 4x4x2 cardboard box. When trash was full the injured lifted the box and set it on 2 feet berm to push it over. Apparently he stepped in loose dirt slope causing him to slip lose his footing backward & overextended rapidly. Afterward, EE felt pain in his calf muscle.
EE was walking to the office to get on duty when he stepped on ice, slipped and fell down on concrete steps hitting his lower left back against the concrete steps.
The injured was trying to re-start stalled but backhoe on scraper haul road. He stated that he climbed out of cab to spray some starting fluid into air filter housing. The injured move quickly to climb back into cab but slipped on the steps and lost his footing that caused to extend his left leg unexpectedly. felt pain after the accident.
EE stepped on uneven floor surface when stepping thru access hole in revolving frame & turning at the same time. He lost his footing & dislocated his left knee. Nothing was broken...put back into place. EE returned to lite duty work on 1-9-06.
Employee was to work under 03 belt to patch up concrete but first went to get a hose to wash down the area. Trying to connect the hose to the coupling, he slipped and fell to the ground, hitting the inside of his thigh against the concrete.
The operator was dismounting his equipment when his right foot slipped off the step. He fell down twisting his right ankle onto an uneven ground surface, injuring it.
The oiler was kneeling on the propel shaft on the dragline, tightening a nut on the intermediate shaft bracket-bolt when the propel shaft rotated, causing him to lose balance and fell forward hitting the compartment wall with extended left arm. He dislocated left shoulder when he collided with the wall.
The serviceman (greaser) was helping a welder install a handrail on a dozer when he grabbed an unsecured rail causing him to lose his balance and fell backwards. He landed on concrete floor with both feet.
security officer was washing her unit #306 at the black mesa mine tipple wash rack, while she was walking/moving backwards and fell off the edge of the walkway. she fell backwards about approx. 3ft 9 in. scrape and asmall cut on left elbow officer was treated for her cut at the mine clinic and finish out her shift.
Mechanic claimed that on 9-9-03 around 8pm, while replacing dozer trans. felt his injury occurr. at time a veryslight pain was felt which he thought wasn't significant to warrant immediate attention. 9-10-04 4:30 pm, he was walking thru the shop and turned left, felt a sharp pain shott thru his right knee.
The injured was dismounting 8750 dragline when he stepped on a rock, causing him to lose his balance. he fell backward twisting his left foot, his knee hit other small rocks on the ground causing a fracture to his left kneecap.
THE PREP LABOR WAS PUTTING TOOLS BACK INTO THE TOOL BOX WHEN HE TRIPPED ON TOOL BOX FLOOR EDGE OR LIP OF BOX AS HE TOOK A STEP BACK. HE FELL STRIKING HIS HEAD AGAISNT THE CAROUSEL BRACKET.

THE INJURY WAS CLIMBING BACKDOWN FROM SNOWPLOW SANDBOX TOWARDS CAB OF SNOWPLOW DRIVER DOOR, USING HANDRAILS AND STEPS, WHEN HE APPROACHED SNOW-PLOW CAB SIDE DOOR HE GRABBED PARTIALLY OPENED DOOR WINDOW. THE DOOR SUDDENLY SWUNG OPEN AND HIT THE INJURED IN THE RT. FOREHEAD CAUSING A SMALL ABRASION.
OFFICER WAS ATTEMPTING TO CLOSE AND LOCK THE PUBLIC COAL LOAD-OUT FACILITY, LOT A LEFT GATE. LEG WAS STUCK IN THE GROUND AND OFFICER PULLED ON THE GATE, HAND SLIP, & CAUSED OFFICER TO FALL BAC KWARDS, FELL & HIT HIS HEAD ON GRAVEL ROAD ROCKS& SUSTAIN SMALL CUT ON BACK RIGHT SIDE OF HEAD. OFFICER GOT DIZZY FOR A SHORT WHILE & GOT A HEADACHE.
THE MECHANIC WAS CLIMBING DOWN OFF A DOZER WHEN HE LOST HIS FOOTING CAUSING HIM TO FALL BETWEEN THE MACHINE AND THE ROLLER FRAME. HE HIT THE RIGHT SIDE OF HIS RIBS AGAINST THE ROLLER FRAME AS HE FELL TO THE SHOP FLOOR.
STARTED UP LADDER ON SIDE OF TRUCK WITH 5 GALLON GAS CAN IN HAND. WHEN STEPPED UP TO SECOND STEP HE LOST HIS GRIP CAUSING HIM TO LOSE HIS BALANCE AND FALL. HE FELL ON UNEVEN GROUND TWISTING HI S KNEE.
AT ABOUT 0840 ON 8-24-2000, THE INJURED WAS UNLOADING STRAW BALES IN OUR YARD AT THE OLD BUCKET SHED FROM STRAW SUPPLIERS TRUCK. INJURED EE FELL APPROX. 15' FROM TOP OF BALES STRACKED ON TRUCK , LANDING ON RIGHT SHOULDER AREA. MINE NURSE WAS NOTIFIED AND RESPONDED WITH AN AMBULANCE, TRANSFERRED TO CLINIC THEN ON TO CITY BY MEDICAL AIRPLANE.
INJURED STATED THAT HE STOPPED HIS EQUIPMENT FOR THE SERVICEMAN TO FUEL. AFTER FUELING, THE OPERATOR DECIDED TO TAKE A BREAK. HE OPENED THE RIGHT SIDE DOOR AND PROCEEDED TO DISMOUNT THE EQUIPM ENT. HE FELL TO THE GROUND LANDED APPROX 4 FT FRAWAY FROM DOZER BLADE ARM. THIS CAUSED BRUISING TO HIS SIDE RIB AREA AND A LACERATION TO HIS FOREHEAD.
EE WAS LEAVING HIS WORK AREA, END OF THE SHIFT, HE STEPPED THROUGH AN OPEN MANWAY THAT IS HINGED. THE MANWAY WAS LEFT OPEN BY SOMEONE ELSE AFTER HE WENT INTO HIS WORK AREA.
THE WELDER WAS WORKING ON HAUL TRUCK TRAILER SETTING METAL PLATE ON BACK OF TRAILER, WHEN HE MISJUDGED STEPLADDER HE WAS WORKING OFF AND FELL APPROX. 3' HITTING HIS HEAD ON CONCRETE FLOOR.
THE WELDER DECIDED TO CRAWL ACROSS THE HOPPER-GRIZZLY DURING REPAIR. HE TRIPPED AND FELL FORWARD STRIKING HIS LEFT HAND AGAINST THE SIDE OF THE HOPPER-GRIZZLY.
