Showing 50 of 70Employee was in the process of cleaning out a shelter hole along the track haulage. His left foot was placed on a coal shelf on the mine bottom by the shelter hole. As he reached down to pick up a large piece of coal toward the back of the shelter hole, his left foot slipped causing him to hyper extend his knee. The employee started missing work on 10/10/09 due to this event.
Employee was walking down the steps at the Prep Plant. She slipped on the last step twisting her left knee. She has continued to work until 10-2-09 when she started missing work for a torn meniscus.

He was being evaluated by a doctor until June 20, 2008. On June 20, 2008 he was evaluated and diagnosed as being unable to return to work.
On 4/30/2008 the employee was pushing over an old cribbing that was set. In the process, he stepped on some material and slipped twisting his right knee. He has continued to work until 7/14/2008 when he started missing due to having surgery on his knee.
On 3/17/2008 the employee was stepping down the steps of a ladder on a belt cross-over. His foot became caught in the step causing him to fall to the bottom twisting his back. He continued to work until 3/26/2008 at which time he started missing due to back pain.
Employee was crawling across a conveyor belt cross-over platform. The left handrail broke causing the employee to strike the left side of his face on the frame of the platform.
Employee was helping to hose down the floor surrounding the stacker belt tailpiece. As he went to shut water hose off, he slipped on the expanded metal flooring straining his right shoulder and left knee. Employee has continued to work until 4/21/2008 when he started missing work due to pain from his injuries.

EE fell while cleaning bottom of thickner at the prep plant. He strained his left shoulder in the process. EE started missing work on 9/10/07 due to left shoulder pain.

Employee was exiting a motor as he steped out the motor moved causing him to fall. EE felt pain in lower back and shoulder.
EMPLOYEE CLIMBED UP ONTO A SUPPLY CAR TO UNHOOK CHAIN, AS HE STEPPED DOWN HE LOST HIS BALANCE & FELL FROM THE SUPPLY CAR.
EMPLOYEE WAS STEPPING DOWN FROM OVERDENSITY PUMP, FELT PAIN IN RIGHT KNEE/HIP.

EMPLOYEE WAS CLIMBING DOWN LADDER FROM A FRONT LOADER AND MISSED THE BOTTOM STEP. HE FELL TO THE GROUND TWISTING HIS LEFT KNEE. HE WAS DIAGNOSED BY PHYSICIAN ON 3/31/04 FOR AN ACL TEAR ON LEFT KNEE.
EMPLOYEE PUSHING CONCRETE BUCKET, TRIPPED OVER A TIMBER AND TWISTED KNEE.
EE WAS DIAGNOSED WITH A TORN MENISCUS IN HIS RIGHT KNEE WHICH HE ALLEGES TO BE WORK RELATED.
