Employee was stepping out of a locomotive when they felt pain in their lower back. This is a recurrence of an injury suffered at Emerald Mine (3605466) on 5-20-2010. The employee has been treated for lower back pain since then and was taken back off work by their Doctor.

The employee was walking across the track when EE tripped over the rail. The employee fell and caught self with EE's right arm and felt pain in EE's right shoulder. The employee received an injection in EE's right shoulder on 3/7/2016. This is a Medical Treatment accident.
The employee reported feeling pain in his left knee. The employee was given an MRI which revealed a torn meniscus and multiple degenerative conditions due to repeated trauma over many years of working in the mine. This is being reported as an occupational illness. The employee continued to work until his surgery on 12/23/2015.
At Surface mine. Installing 10oz.felt and 40 mill plastic felt for impoundment. He was pulling plastic down the slope and at the bottom of the slope he went down. Stepped on uneven ground which made him fall. fractured left tibia and fibula.
Dragging a rockdust hoseout thru a xc, employee stepped in a hole hidden by rockdust and twisted his right knee. THe employee continued to work till 01/13/2016 when he had surgery for a meniscus tear. His first missed day was 01/13/2016.

EE was in #9 shop getting out of 5 ton when Rt leg got caught while getting out. EE tired to catch himself with Lt foot and heard a pop in Lt ankle. EE Transported to Whs-Greene where he was found to have broken his Lt ankle in two places. This is being reported as a LTA (NFDL)
Muddy walkway caused miners foot to stick in mud; while pulling foot free miner strained his hamstring.
Traveling down Slope Belt, EE slipped on loose material on concrete bottom. When falling, EE grabbed cradle with LT hand & swung around. EE's RT hand glanced off Belt Crossunder & got caught between belt and Top Roller, then RT hand got caught between crossunder and the belt. EE removed hand. EE received 5 sutures to hand. This is being submitted as a Medical Treatment (NDL).
While carrying supplies, EE twisted ankle in ruts and fell to her knee, striking a rock. This was originally reported as a report only. It was brought to the attention of the safety department that the EE has started physical therapy and this incident is now being reported as a NDL (medical treatment).
EE was at Cumberland Portal attending safety training. During class break he was walking to the rest room located next to the training room. EE was conscious, alert, first aid administered, transported to ER. Small laceration required 2 staples. Attached medical information show dizziness caused by syncope. No act, no condition please consider OI.
EE was pre-op prepping Hyster forklift. He slipped on ice, landing on left side. 3.10.15 diagnosed w/rotator cuff inj. by Dr. taken off work. Previous left rotator cuff repair on 3.28.01 MRI revealed moderate atrophy of supraspinatus & infraspinatus tendons & moderate atrophy of respected muscles. we believe prior Sx contributed to most recent tear & submit as occupational illness.
EE was walking from his car to the guard shack when he slipped on the ice and fell, striking his head on the gravel, just above the Lt eye. EE received 6 sutures. EE will not miss any work. This is being reported as a medical treatment (NDL). Incident happened on a Saturday and EE returned to work on Monday.
Walking through water, stepped in a hole which was not visible. Felt pain in lower back. EE requested and made a chiropractic visit on 12/29/2014. This report being made due to incident considered a medical treatment.

EE was in the process of showering after shift, left shower head to get some gojo from dispenser near shower entrance. EE's foot slipped off edge of floor into drain trough. Caught top of foot on grating. EE received 6 sutures and a broken 2nd metatarsal with complete tendon disruption. EE will be having surgery on 11/21/14.
EE tripped over hose and coal chunk causing him to fall and land on lt elbow and striking rib cage. Incident was originally reported as first aid. EE has started chiropractic care and is now being reported as a medical treatment. EE is still working and has missed zero days of work.
When exiting CM, EE stepped down off CM onto a rut with Rt foot. EE twisted Rt knee and fell striking Rt knee on a rock. EE went lost time 9/2.

Employee was in 63HG walking between 2-1 Overcast, crossing under between the Overcast. Employee stepped on uneven surface and twisted his knee.
EE stepped in dip in mine floor that was hidden with a coat of rockdust, causing pain to his RT knee. EE continued to work regular job. On 4/18/14, MRI showed a horizontal meniscus tear & degenerative changes in RT knee. With medical information, no traumatic injury and performing normal duties, this will be reported as an Occupational Illness. First day off work is 5/7/14.
Lost balance while lifting ties.
Preparing barge for loading, movement of barge caused EE to lose balance, falling approx. 12' into empty barge. Ladder was provided & EE was to climb out of barge. EE sustained fractures to L wrist, R hip & lacerations to forehead. 7000-1 report is being filed as an excise of caution and does not constitute an admission that MSHA has jurisdiction over this injury.
The ee exited his vehicle when arriving at work & when he turned to reach for his dinner bucket that was still in his vehicle, he fell to the ground, causing pain to right knee. MRI on 1/20/14 showed advanced degenerative change in his right knee. The ee's 1st day away from work was 1/20/14
Climbing down a ladder in the #6 elevator from the top of the elevator after making an inspection and his leg buckled causing him to fall off the ladder and strike the floor. He strained his right shoulder and fractured his left ankle. He began losing time on 12/16/13.
Employee tripped catching himself with his right hand causing him to fracture thumb.
Getting off 9 ton motor and stepped on ground and left foot rolled and he fractured his ankle.
On the belt drive cutting a bed rail and the rail broke off. He slipped off the drive and struck his face on the drive grid lacerating his chin. The cut required eleven sutures to close. He returned to work his next scheduled shift missing no work. This is a medical treatment injury. This employee is a weekend/holiday employee and his next scheduled shift was 6/22/13.
While examining gob vent borehole 59-2 he was approaching the site and stepped over a three-eights inch cable to gain access and tripped on the cable. He fell and fractured his right wrist and received a severe contusion on his left knee. The cable is used in lieu of a gate.
Lifting a bonder out of scoop bucket, stood bonder up and bonder turned. The wheel rolled out from under it and he was pulled into the scoop striking his right knee on the scoop bucket. He received a severe contusion on the right knee.
